NM Mi Via Self-Directed Waiver The NM Mi Via Self-Directed Waiver is designed for individuals with developmental disabilities or those who are medically fragile. This waiver allows participants to create personalized service and support plans (SSPs) with the aid of chosen consultants and support circles. It promotes a person-centered approach, ensuring that services cater to individual needs and preferences. Eligible individuals must have received an allocation from the Developmental Disabilities Waiver or the Medically Fragile Waiver. The program is administered by the New Mexico Health Care Authority and is approved under the 1915(c) authority.
