Guardiant Health works with managed care entities on post discharge medical care program with an assigned medical team that works with patients coming out of the hospital and the complex needs that follow. We have anecdotally noticed these trend in home health as well and fortunately have been able to mitigate this gap through our post discharge medical team. Its not like MA insurers didnt see this coming . Unfortunately Humana is not alone as many plans have been laser focused mostly on HCC/ RAF coding to boost top line revenues for years and failed to plan on the transition to STAR ratings which if missed, have longitudinal impact on the plan beyond HCC/ RAF coding shortfalls! In earlier conversations with MA plans , focus on STAR ratings was a key capability we developed at Guardiant Health but I was always amazed at the lack of concern or urgency - hopefully plans will now see the need but it will take them time to turn the ship on this. My concern is that plans may shed poorly rated plans and exit counties to mitigate their potential loses and Medicare enrollees will bear the brunt. #medicare #stars #medicareadvantage

Hospital readmission rates have come down significantly due to increased awareness, incentives, penalties and improved coordination of care efforts but much can still be done to further improve outcomes as well as a better patient/ caregiver experience, This program offered under Guardiant has shown significant improvement in overall readmission rates for a medicare/medicaid population under the care of a large managed care medical group What is it that made such an impact? Its not rocket science but you need a system and model designed to consistently engage the whole person needs that patients face after an acute hospitalization. #readmissions #medicare #medicaid #patientexperience #proactivecare
