Our care coordinators are where the rubber meets the road with the work we do at GroundGame Health, and in our From the Field series, we highlight the stories about the people we serve, as told by the care coordinators who helped them. In this installment, care coordinator DeAndre talks about someone whose needs were initially centered on the types of fundamentals that we work diligently every day to solve. But once those needs were met, DeAndre discovered that their ability to live independently was on the line. He worked to find a creative solution that made a real difference. Abigail provided her with the support and help she needed by coordinating and arranging for resources provided by organizations in her community.
