Freedmen’s consulting team has been in the forefront of implementing value-based models of care in support of the U.S. Department of Health and Human Services, Administration for Community Living (ACL). The team was initially focused on the body of consulting work produced by Timothy McNeill and has now grown to a team of experienced project managers with experience in medical practice management, ACO management, bundled payment programs, and integration of community-based organizations to address medical risk and social determinants. Our team specializes in the delivery of technology-enabled care management that includes predictive analytics, artificial intelligence, and Military-Grade Medical Logistics.
