Forward Horizons Inc

Our mission is to support those with disabilities who seek our services in their pursuit of full citizenship, which we do by helping develop mutual collaborations, honoring the role of each person within their community, and respecting their diverse abilities, hopes, and dreams. Forward Horizons serves adults and youths with disabilities in Ashland and Bayfield counties in home and community based settings. We are an all local, grassroots organization; female-founded and presently women-managed. We value self-determination, innovation, compassion, fun, and person-centered planning for our clients. Our Direct Support Professionals provide several services from a variety of referral sources: IRIS, Inclusa, and Children's Long Term Support (CLTS). Those services include Supported Home Care, Community Based Day Services, Respite, and a few kinds of Transportation (Specialized, Medical, Non-medical, and Hospital Discharge). We also provide contracted Support and Service Coordinator (case management) for the Bayfield County CLTS program. We encourage those seeking our services to please connect with the ADRC offices in Ashland or Bayfield Counties, or if an individual already works with a case manager, please contact them to learn about the services that are available. On June 24th, 2020, the Forward Horizons Board of Directors, Administrative Director, and Program Director issued a public statement stating this organization's solidarity with the Black Lives Matter movement.
