The results of the 3-year outcomes of treatment with the GLP1/GIP dual agonist tirzepatide are out, and the data are impressive and impactful. At 176 weeks, the study participants with both?obesity and prediabetes on the?15-mg (highest) dose?lost an average of 19.7% of their weight. Besides some GI side effects, there were no major safety signals. And the delay in progression to type 2 diabetes from the high risk state of prediabetes was incredible. In the tirzepatide group only 1.3% progressed over 176 weeks, compared to 13.3% in the placebo group. A 93% reduction in risk, in just 3 years of follow up. It is a huge leap forward to have clinical tools that address both obesity management and diabetes prevention so effectively.?A markedly lower progression to type 2 diabetes has incredible clinical impact for individuals. It also has important economic implications. Type 2 diabetes is one of the most costly diseases. Medical expenses from employees with diabetes are about double compared to those without, and it is estimated US?employers lose more than $20 billion annually due to unplanned missed days of work by?employees with diabetes.?So although this study did not look at ROI, these data are critical as we build the case(s) for the imperative to treat obesity?-- as a way to prevent, and avoid the innumerable costs, of cardiometabolic disease.

There is a subset of patients considered “non-responders” to therapy in clinical trials; however, I propose we look at it from the perspective of *the therapy is not working* for this patient (rather than the patient is not responding). So why does this occur even with these so-called “miracle drugs?” It all boils down to the fact that there are important differences among individuals that will effect outcomes of treatments for obesity, whether that be lifestyle changes, medications or surgery. We cannot forget that we are treating individuals and need to be versatile to utilize other tools in the toolbox to find what works with this unique patient. This article also reminds us to consider whether the clinical trial population is or is not similar to the individual patient being treated. In the future, I believe we will have better ways to identify the best treatment for each individual. Until we have better guidance, remember that if the treatment is not working for the patient, don’t be afraid to make a change, and let’s support the patient with the knowledge that it is not their fault if a medication is not the right fit. https://lnkd.in/gK2PiSwE

The Obesity Society's #ObesityWeek 2024 was so energizing and inspiring! I was so grateful to speak on the Obesity (R)evolution panel moderated by Dr. Deborah Bade Horn, alongside Drs. Terry Wickham and Sarah Ro. I was especially inspired by Dr. Ro's work scaling access to obesity care in an underserved rural patient population, mostly covered by Medicaid. Dr. Ro has taken such an innovative and creative approach in the University of North Carolina health system, and I found so much alignment with what we do at Form Health. ? Severity/Risk stratification. Her (and her team's) model uses obesity severity to triage patients to different access points and interventions. They successfully serve all patients, not just those who have coverage for GLP1 receptor agonists, by combining effective science-based lifestyle support and prescribing across a range of pharmacotherapy that can support weight loss and metabolic health. In fact, the top two medications used are #metformin and #topiramate. We too have seen that using a broad spectrum of medications can drive impactful health gains and in a more cost-controlled way. I am going to steal a "clinical pearl" from her: While we all know that newer AOMs are more effective on average than older meds, she helps patients on the lower end of the severity spectrum feel optimistic about the health results they can accomplish by counseling that you don't always need to "shoot a fly ?? with a cannonball ??" -- with non-GLP1 options we can still see impactful results. I loved this! ? Leveraging telehealth. Her program offers in person and telehealth visits, so that patients can have greater flexibility and engagement. The patient case she presented was someone who started out with very little mobility, difficulty getting to appointments and unable to work. It was so inspiring to see a photo after treatment, of the patient standing next to Dr Ro in an elegant dress, now able to work thanks to weight loss driving such health improvements! ? Ongoing clinician education and learning. Dr. Ro won institutional support to cover ABOM certification for clinicians and help them deepen their expertise and serve patients more effectively. She highlighted that a greatly appreciated perk for these clinicians is an ongoing monthly educational conference. At Form, our clinical team gets together virtually once monthly for a journal club and once monthly for a clinical case conference. With our our commitment to evidence-based practice and our field changing so rapidly, this is so important. These conferences --where I learn from my amazing colleagues-- are also a top highlight for me personally! It's so exciting to learn what others are doing to expand access to effective obesity care. Form Health physician, Lauren Lemieux, MD, FACP, DABOM, presented new findings at #ObesityWeek presented by The Obesity Society. The study was completed in collaboration with researchers at David Geffen School of Medicine at UCLA and demonstrated improved weight loss outcomes from Endoscopic Bariatric Therapy (EBT) when paired with specialized care from a telehealth obesity clinic. The prospective observational study found that patients referred to Form Health after EBT achieved greater weight loss at 12 months compared to those who did not receive teleheatlh obesity medicine follow-up care. Increased weight loss likely resulted from the specialized obesity care provided by an ABOM-certified physician and a CSOWM-credentialed Registered Dietitian. This dedicated Care Team created a personalized plan for each patient that addresses behavioral changes, nutrition, physical activity, and FDA-approved medications, if appropriate. These findings underscore that obesity is a complex, chronic disease requiring comprehensive and individualized care for lasting success. One procedure alone is rarely sufficient—sustained weight loss often requires ongoing treatment leveraging other tools like lifestyle intervention and FDA-approved medication. Form Health physicians Beth Marshall, DO, Dipl. ABOM and Florencia Halperin presented their findings “Real-World Analysis of Semaglutide Outcomes in a Telehealth Obesity Medicine Clinic” at #ObesityWeek presented by The Obesity Society. Their analysis demonstrated that virtually-delivered intensive lifestyle therapy combined with semaglutide led to clinically meaningful weight loss of 8.4%, 13.6%, and 17.7% at 6, 12, and 18 months, respectively.
