We know Ambulance Revenue Cycle Management isn't all glitz and glam but sometimes it gets exciting...especially for those who proudly call ourselves "billing nerds"! Have you heard about the modification to the ALS2 (HCPCS A0433) service level? Prehospital Blood Transfusion (PHBT) is being added as a qualifier for ALS2!!? ? The following announcement is from CMS: ? After consideration of public comments, and considering further that the indications for use, storage requirements, administration, monitoring and training for the use of PRBCs and plasma are similar to those previously described for WBT, we are finalizing our proposal to modify the definition of ALS2 at §414.605 by adding the administration of prehospital blood transfusion (PHBT) which includes low titer O+ and O- whole blood transfusion therapy, PRBCs, plasma, or a combination of PRBCs and plasma.?A ground ambulance transport that provides one of these PHBTs would itself constitute an ALS2 level transport. ? This is very exciting news for ground teams providing these costly services, as now it can be qualified for a higher level of reimbursement. Previously, according to Chapter 10 of the Medicare Benefit Policy Manual, in order be considered for reimbursement at the ALS2 level, the ground service had to provide:? ? At least three separate administrations of one or more medications by IV push/bolus or by continuous infusion (excluding crystalloid fluids) ? -or- ? The provision of at least one of the ALS 2 procedures listed below: Manual Defibrillation/Cardioversion Endotracheal Intubation Central Venous Line Cardiac Pacing Chest Decompression Surgical Airway Intraosseous Line ?
