Happy Veterans Day! ?? ?? Thank you to the brave men and women who courageously serve to protect our country, including one of our own, Theresa Linder! Theresa served in the Airforce as a fire fighter in an EMT for 11 years. Theresa says she would have served even longer if it weren't for her knee injury she suffered while fighting a fire. After her retirement she enrolled at Rochester Institute of Technology where she received a Bachelor's Degree in Diagnostic Medical Sonography, and then joined the team at EZaccessMD and UltraMobile Imaging as a Medical Sonographer performing ultrasounds for patients in their homes. What is one diagnostic portable radiograph that can help make significant difference for the dialysis patient? A simple chest x-ray done at the place of residence. On this example, a chest X-ray can help detect fluid overload in dialysis patients by showing signs of excess fluid in the lungs and heart. Here’s how it can be helpful: 1. Detecting Fluid in the Lungs (Pulmonary Edema): One major sign of fluid overload is pulmonary edema, where fluid builds up in the lungs. On a chest X-ray, this can appear as denser areas around the central lung region. In more severe cases, it creates a “bat wing” pattern, showing fluid filling the small air sacs (alveoli) in the lungs. 2. Enlarged Heart (Cardiomegaly): When fluid overload becomes chronic, it can increase the heart's workload, causing it to enlarge. While a single X-ray can't prove chronic fluid overload, multiple X-rays over time that show heart enlargement can indicate it. 3. Fluid Around the Lungs (Pleural Effusion): Fluid from overload can sometimes collect around the lungs. This appears on an X-ray as a change in the normal shape at the lung edges or as visible fluid layering in the pleural spaces. 4. Timing for Accurate Diagnosis: The timing of the X-ray is important. It’s most helpful if done when the patient shows symptoms of fluid overload, like shortness of breath or a cough. Additionally, comparing X-rays taken before and after dialysis can help determine how much fluid was removed. 5. Ongoing Monitoring: For patients with frequent fluid overload issues, periodic chest X-rays can be helpful for early detection, especially if their symptoms aren’t clear. Though a chest X-ray can reveal a lot, it’s most effective when combined with other indicators like weight changes, blood pressure, specific lab tests (like BNP), and physical exams to fully assess fluid status in dialysis patients.
