Everyday Life has had the unique opportunity to work with health and human service organizations in and outside of Michigan which has allowed us to gain insight and build on promising practices. Areas of expertise include but are not limited to; Community Health Workers (CHW), practice transformation, tri-sector partnerships, social determinants of health, change management, community systems integration and Clinical Community Linkages (CCL).

  • We are excited to share we are supporting the Neighborhood Wellness Center initiative as a key partner with Wayne State University’s Community Health Worker Academy. The goal is to enhance workforce and social services provided by Community Health Workers by launching consultations with community residents, conducting social determinants of health screenings, providing health education and healthy living resources, offering social service referral opportunities, and continuing follow-up visits with residents. "Meeting people where they are at," to ensure community members can access the supports needed to improve living conditions, prevent the most pressing chronic diseases, and advance health equity across Michigan. https://lnkd.in/gsExyEeF

    It was an honor to visit the Neighborhood Wellness Center at Vernon Chapel A.M.E. Church in Detroit alongside Lt. Governor Garlin Gilchrist II?and Rev. Sharinese Jackson. The Michigan Department of Health & Human Services is investing $17 million in 22 Neighborhood Wellness Centers across the state. The centers offer free preventive health care services, such as blood pressure and diabetes screenings, and substance use disorder prevention services. In addition, on-site community health workers support individuals by identifying social determinants of health needs, such as food, transportation and housing, and linking people to resources to address those needs. I’m grateful to Rev. Jackson and local leadership at all of the Neighborhood Wellness Centers for their partnership, and I look forward to continuing to work together to reduce health disparities and make sure individuals have access to health care where and when people need it.
