They're going to nickel and dime you to death...but there IS a solution... Let us explain... Spread pricing is unfortunately, an extremely common practice for Pharmacy Benefit Managers (PBMs). When they activate this tactic, you or your healthplan is charged a higher price for a prescription drug claim than what they actually pay the pharmacy to fill the prescription. The PBM then keeps the difference between the two amounts which is known as the "spread." For example, if a PBM pays a pharmacy $50 for a drug but charges the health plan $70 for that same drug, the PBM retains the $20 difference as profit. Spread pricing can add substantial hidden costs for employers, health plans, and ultimately, patients. But now for the good news... You have a better alternative... ?? ????????-?????????????? In this model, the pharmacy benefit manager (PBM) charges you exactly what the pharmacy is paid for the prescription drug, with no hidden markups or price differences. There's an administrative fee built in that is transparent and often rebates are also passed through. Large PBMs often steer members toward more expensive medications, pocketing bigger profits through rebates, which means you're stuck with higher bills. Ugh. They charge you more for prescriptions than they pay pharmacies, keeping the difference—a hidden markup known as "spread." Even worse, PBMs negotiate rebates with drug manufacturers but frequently retain those rebates for themselves, sometimes hiding them offshore. Additionally, PBMs create revenue streams from manufacturers through “access fees,” and many own their own specialty and mail-order pharmacies, leading to conflicts of interest that drive up costs. While they may advertise low administrative fees, hidden charges often lurk in the fine print, adding significant extra costs for clients.
