Since moving to Illinois Veteran’s Home – Quincy in March 2022, Al has faced mobility challenges due to Parkinson’s disease and COPD. But with resilience, hard work, and the dedicated support of PTA Krystin in Physical Therapy, he’s made incredible strides. Using a U-step style walker, Al can now walk 100 feet—a huge milestone in his journey. He continues to build his strength through an ongoing exercise program, staying active and independent. When Barbara first began therapy, she relied on an electric wheelchair and struggled to walk even 50 feet. After multiple falls, she required the highest level of care in her community. Following a serious fall in October, she feared she might have to move out. But Barbara never gave up. Through dedication, hard work, and the support of her therapy team, she has made an incredible transformation. Today, she walks to all her meals using a 4-wheeled walker and can even navigate small spaces without assistance. She has lowered her level of care, re-engaged in community activities, and is once again enjoying outings with old friends. Beyond her progress, Barbara has been a ray of fun and positivity in the clinic, bringing energy and joy to everyone around her. EmpowerMe 360 in Action: Timely Care, Right at Home When Carole, an Independent Living resident at Township Senior Living, enrolled in EmpowerMe 360, she didn’t expect to need urgent medical attention—but thanks to having a daily on-site team, she received the care she needed, exactly when she needed it. During her enrollment visit, EmpowerMe therapist Shannon Rook noticed a concerning wound on Carole’s knee. Carole shared that she had fallen 10 days earlier, and the wound was now draining and painful. Recognizing the risk of infection, Shannon immediately called on the EmpowerMe nurse practitioner, who assessed the wound, prescribed an antibiotic, and arranged for pickup within an hour—all without Carole having to leave her apartment. With seamless coordination between EmpowerMe’s therapy and medical teams, Carole received prompt care that prevented a worsening infection and kept her on the path to recovery. Steve’s Journey: Strength, Resilience & Independence After an occipital fracture in October 2024, Steve faced challenges impacting his balance, cognition, and coordination. With dedication and help from his therapy team, Carlene Jensen, OTR; Emily Bondarenko, PT; and Antonio Caldwell, SLP, he has made remarkable progress. Physical therapy improved his gait, balance, and mobility, reducing falls. Occupational therapy helped him adapt daily routines for greater independence, while speech therapy strengthened his communication. Cognitive-linguistic exercises further supported memory and processing. Steve’s resilience and commitment to therapy have been inspiring. Stepping Into Independence: A Resident’s Journey Not long ago, this resident relied primarily on a wheelchair to get around. Simple daily tasks felt challenging, and mobility limitations made her hesitant to move freely. But through dedicated therapy with Carina Polanco, PTA and Melinda Francois, COTA/L, she has made incredible progress—now walking independently with a walker, moving effortlessly from the game room to the bistro, and even standing up from chairs without assistance. Beyond mobility, therapy has given her newfound confidence in everyday activities. She now dresses herself using a grabber, gets in and out of the car with ease, and can step into her sister’s garage—something that once seemed impossible. Most importantly, she no longer feels nervous or afraid.
