When patients need care right away, Emcara Health delivers. ?? One of our dedicated Nurse Practitioners, Kim Vuocolo, responded quickly to a call from a patient who was struggling to breathe due to chronic #COPD. Kim scheduled a same-day visit and ordered a chest X-ray, which showed fluid buildup in the lungs and problems with airflow. She acted decisively by adjusting the patient's medications, prescribing a course of steroids, and collaborating with the patient's cardiologist to ensure a well-coordinated care plan. Kim also saw that the patient was overdue for her yearly #echocardiogram and made sure it was scheduled—closing an important care gap. Health needs change over time, and so do the risk scores that determine funding for people in Medicare Advantage plans. Team-based care is a key part of our formula for helping high-needs patients avoid unnecessary hospitalizations. Depending on what the patient needs, we can pull in a physician, registered nurse, behavioral health specialist, pharmacist, physical therapist, or a community health worker. This team coordinates with the patient's other physicians to make sure resources are used efficiently and no one falls through the cracks.
