Amazing day today! Honored to give the opening lecture for the Southeastern section of the American Urology Association (SESAUA) Annual Meeting at the Grand Hyatt Nashville today! My talk was on the management of chronic testicular and groin pain. It’s been 15 years of my life’s work and a topic I am immensely passionate about. This is truly an example of the development of bench research all the way to clinical application. I completed a study 15 years ago identifying specific nerves that may cause this pain and developed some targeted surgical techniques to treat this condition: 1) Office-based Targeted denervation of the spermatic cord (TDSC) & 2) Office-based Targeted peri-spermatic cord & ilioinguinal cryoablation (TPSC). We have completed over 3000 TDSC procedures and over 1500 TPSC procedures now. Overall success rates are: 86% for TDSC & 78% for TPSC. It is a very difficult condition for patients who endure this pain and for urologists to treat. We have helped thousands of patients, but we have also failed to help some patients. Being able to present today, being part of the AUA guidelines committee for some new guidelines for pelvic pain that will be coming out in a few months, and being part of an AUA pelvic pain course at this year’s annual meeting are all a dream come true for me. I really just want to help men with this condition and contribute in a meaningful way. We are always looking for ways to improve our outcomes. ???Toilet Tech: The Power of At-Home Uroflow Monitoring??? Imagine a patient comes into the clinic for uroflow testing. His tracing looks like a textbook-perfect bell curve—strong, steady, and efficient. Based on that single data point, we might assume he's doing well. But what if we had a broader view? Using smartphone-based uroflowmetry, I reviewed this patient’s uroflow tracings over four days. The variability was striking: ???1/31, 4:06 PM?– Beautiful bell curve. Looks great! ???1/31, 8:30 PM?– Still decent, but not as strong as before. ???2/1, 10:20 AM?– Poor flow with a small volume void. Tough to interpret. This illustrates a key point:?uroflow can vary significantly throughout the day. A single in-office test might not tell the full story. If you're not incorporating?at-home uroflow monitoring?for patients with bothersome lower urinary tract symptoms, you're missing out on invaluable?objective data?that can guide decision-making. I use?Emano Flow, developed by?Teddy Hay, PhD and Bishara Korkor, MS, which utilizes a smartphone microphone to analyze uroflow based on the sound of urine hitting the toilet water. It’s non-invasive, easy to use, and provides?insights that a one-time clinic test simply can’t match. The future of urology is here.
