AI CDS is a huge area of interest for many healthcare leaders, but clinicians remain gun-shy, expressing concerns over black-box algorithms, fairness, and patient safety. To that end, the ONC’s new (b)(11) Decision Support Interventions (DSI) criterion (b.11, for short) introduces major transparency, interoperability, and risk management requirements. Here are a couple specific requirements that are especially note-worthy for AI developers and health systems alike: “???????????? ????????????????????” (?????????? ??????????) ?????? ????????????????: AI CDS tools must display 31 key attributes on a model card. This mirrors similar recommendations already released by the Coalition for Health AI (CHAI) and others, reflecting a growing trend toward model transparency; if vendors aren’t already preparing for this, they should. Attributes that must be displayed inside the EHR’s user interface include who developed the model, how the model makes decisions, what data was used for training, known risks and biases, and more. Critically, though, external developers may leave these fields blank (for now), while EHRs must both facilitate their display for all vendors and provide them for EHR-developed AI tools. ???????????????? ???????? ???????????????????? ????????????????????????: If an EHR vendor releases its own AI-powered CDS tool, it must implement an Intervention Risk Management (IRM) program, maintain up-to-date risk disclosures, and publish risk mitigation strategies covering validity, reliability, robustness, fairness, intelligibility, safety, security, and privacy. However, external applications are not (yet) held to the same standards. ???????? ???????? ???????? ???????? ?????? ???????????????????? ??????????????? While this remains conjecture, we see a few potential unintended side effects here: ? While risk management is now a core responsibility for EHRs, there remains a gray area regarding liability if an independent AI tool malfunctions or contributes to a patient safety event. If the EHR simply “hosts” or “passes through” an external model’s recommendations, it’s unclear if they are partially liable under b.11 if the EHR fails to display required disclosures accurately or if an integration error leads to patient harm. ?? ? To mitigate risk, we see a potential outcome where the overhead of compliance will create new “certification marketplaces,” where only AI vendors that have undergone robust b.11-aligned audits are allowed to integrate easily with EHRs. This might increase trust but limit competition. ? With many AI tools built on proprietary algorithms or novel data sets, we also have to wonder how granular transparency needs to be to meet regulatory expectations without forcing IP disclosures. Will vendors be incentivized to provide only the bare minimum such that it's not really useful? So a question for the health system decision-makers: Do these updated guidelines increase your confidence in AI CDS solutions?

Patient scheduling is a massive challenge for providers, with a huge number of vendors working to improve workflows here. In short, patient scheduling is a complex technical problem, because humans are complex. Patient and provider preferences, booking modalities, prioritization, cancellations, and no-shows are all factors a good solution needs to manage. We think about these solutions in four main buckets: ? ???????? like Epic, athenahealth, Oracle, and Healthie ?? ? ????????????????????, ?????????????????????? ???????????????????? ??????????. Many—though not all—of these solutions are specifically designed for administrative staff use, as opposed to patient self-scheduling. Examples include:??Kyruus Health, Zocdoc, Experian, Cal.com, Inc., Cronofy, and DexCare. ?? ? ?????????????? ?????????? ???????? ?????????????????? that handle workflows including scheduling, patient intake and health screeners, two-way communication, and payment processing. Examples include: Luma Health, Clearstep, Notable, Tellescope, Weave, Qure4u, and Fabric. ?? ? ?????????????? ???????????????? using AI agents to help manage patient appointments and scheduling without administrative intervention.
