Elder Care Management provides guidance and support for the care providers struggling to get through the day and the people that they love. Our care managers guide families through a maze of complicated choices and remain with them to oversee and support them on their journey. Our staff has worked in the senior care community for over 17 years providing hundreds of families with education and creative problem solving. Our services include: An initial free phone consultation Care plans and assessments. Safety evaluations. Attend physician’s appointments and advocate during emergency room visits or hospitalizations. Act as intermediaries to help families reach mutual consensus. Oversee clients care in their home, assisted living communities, skilled nursing. Advocate with health and industry professionals to ensure quality care. Relieve family members of the day to day demands of care.
