For today’s #55for55 feature, we’re taking a closer look at the SDMC: ?? Empowering Seniors Through Advocacy: The Surrogate Decision Making Committee (SDMC) ?? At EAC Network, we believe every individual deserves dignity, respect, and proper care, regardless of their physical or mental capacity. That’s why our Surrogate Decision Making Committee (SDMC) ensures informed, compassionate medical decisions for those in need throughout New York City, Long Island, and Rockland County. ?? The Problem: Individuals unable to provide informed consent due to physical or mental challenges can be easily taken advantage of and may be at risk of receiving improper care. ?? Our Solution: The SDMC coordinates hearings for non-emergency medical and dental treatment for these individuals, advocates for their best medical interests, and provides a timely, person-centered approach that’s cost-free and easy to access. ?? How It Works: A four-member panel consisting of an attorney, medical professional, family member, and an advocate, ensures: 1? Dignity and participation in the hearings for the patient 2? Timely access to hearings and major medical treatment 3? Life-changing outcomes through appropriate medical interventions ?? The Impact: Through SDMC, at-risk individuals gain access to timely care and a team dedicated to protecting their rights and well-being. One week until Giving Tuesday! For today's #55for55, we're introducing you to E-MDT: the Suffolk County Elder Abuse Enhanced Multidisciplinary Team. This EAC program protects our seniors from abuse, neglect, and exploitation. It provides a support network of professionals to respond to and report the victimization of older adults, while increasing their access to resources, services, and justice. With a trauma-informed, collaborative approach, these groups of professionals provide a lifeline for adults aged 60+ who may be experiencing physical, emotional, or financial abuse. How we help: ? Bringing together expert agencies like law enforcement, social workers, healthcare professionals, and legal advocates to provide support. ? Addressing elder abuse cases to ensure safety, justice, and stability. ? Offering creative, compassionate solutions tailored to each individual’s needs. It’s time for #55for55, and today we’re showcasing one of our most active programs! EAC’s Senior Health Promotion plays a vital role in enhancing the lives of seniors by providing opportunities for physical activity, creative expression, and social connection. We provide activity specialists to senior centers across Nassau County to lead a diverse range of wellness classes, including: ?? Tai Chi and Yoga for balance and relaxation ??? Chair Aerobics and Zumba for movement and cardio health ?? Music Therapy and Line Dancing to lift spirits and engage minds ??? Ceramics, Creative Writing, and Arts & Crafts for creativity and self-expression For added accessibility, the program features two weekly Zoom classes. It also extends beyond these activities by providing health and nutrition screenings, helping seniors maintain overall well-being. The ultimate goal is to empower older adults to stay active and independent, both at home and in their communities. Participants often share how much better they feel after engaging in these activities, highlighting the importance of not only physical exercise but also the sense of community fostered through the program. As a seasoned business leader and advocate, Sammy Chu brings a wealth of experience and passion to this fight. As CEO of Edgewise Energy, a NY-based clean energy development firm, Sammy is driving innovation in sustainability. Under his leadership, Edgewise Energy manages one of the largest community distributed generation projects in the country, saving Long Islanders over $100,000 monthly on electric bills while significantly benefiting the climate. Sammy’s dedication to Long Island extends beyond business. He previously served as Chief of Staff and Commissioner for Suffolk County’s Department of Labor under County Executive Steve Bellone, where he led recovery efforts after Superstorm Sandy. He also serves on multiple boards for non-profit agencies, including EAC Network, the New York League of Conservation Voters, and the US Green Building Council’s Long Island chapter.
