"Pain management has come so far in the last few years!" Instead of simply repeating this sentiment everyday, I am going to take it upon myself to educate my colleagues, referring physicians, patients, and everybody else on LinkedIn exactly what it is that I can do in my practice at Florida Spine Care. Each week I will be sharing an exciting and effective pain intervention that you probably did not learn about in medical school. My posts are going to be short and to the point and SUPPORTED BY DATA!!! I'm going to kick it off with one of my favorite procedures; dorsal root ganglion (DRG) stimulation. The DRG is a collection of sensory nerve cell bodies located just outside of each level of the spine. It has been recently identified to be heavily involved in the development of chronic neuropathic pain. DRG stimulation is a neuromodulation technique used to treat focal neuropathic pain. The landmark ACCURATE trial (attached below) demonstrated superiority of DRG stimulation to traditional spinal cord stimulation for the treatment of CRPS. I have personally seen it successfully mitigate the pain associated with other painful conditions, including post total joint (hip/knee) replacement pain, phantom limb pain, post-herniorrhaphy pain, chronic pelvic pain, and other focal neuropathic conditions. What I love about DRG stimulation is that you get to trial it before committing to the therapy! A trial involves placing the stimulating leads in place under fluoroscopic guidance in my office and attaching them to an externalized wearable battery. After a few days, the patient decides whether this therapy is right for them. If so, I place the same type of leads in an operating room tunneled to a subcutaneous implantable battery-- an outpatient procedure with minimal recovery time.
