"Big Data Bayesian Truths: No Vancomycin Trough Concentration Target is Sufficiently Precise for Safety or Efficacy" ???? Authors: Justin S., Sharmeen Roy, PharmD, BCPS, Paul Sabourenkov, Marc Scheetz ?? A large-scale, international study (17,711 patients across the U.S., EU, and Australia) reveals major inconsistencies between vancomycin trough concentrations (Cmin) and true AUC targets. These findings reinforce the need for AUC-guided therapeutic drug monitoring (TDM). ?? Key Findings: ? Only 23% of paired trough and AUC values were within the target range. ? Categorical disagreement between Cmin and AUC was 34.3%. ? 30% of low trough levels (Cmin) still had target AUCs (400–600 mg × 24h/L)—leading to unnecessary dose escalations and increased nephrotoxicity risk. ? No clinically used trough range consistently predicted AUC targets with precision. ?? Why This Matters: ?? Supports 2020 IDSA/PIDS/SIDP vancomycin guidelines recommending AUC-based monitoring over traditional trough-based methods. ??     A huge thanks to our coauthors on this (tagged below). The alternate title could have been, "Troughs are not enough to provide safe care to patients receiving vancomycin." Data from this paper dispel the commonly held belief that troughs are precise enough estimates of AUC. Since AUC is clearly linked to kidney injury, making sure you do not go over your target AUC is critical to patient safety.
