    Why-Why Analysis (Root Cause Analysis) Purpose: A problem-solving method used to identify the root cause of an issue by repeatedly asking "Why?" until the fundamental cause is discovered. Steps: 1. Ask "Why?" for the problem or symptom. 2. Ask "Why?" again for each answer until the root cause is identified. Example: Problem: Defective products on the assembly line. Why: Machine issues. Why: Machine not calibrated. Why: Calibration schedule missed. Why: No reminder sent to maintenance. Why: Reminder system down. Root Cause: Reminder system failure caused missed calibration, leading to defects. Outcome: Fix the reminder system to ensure timely calibration and prevent defects.
