Looking forward to 2025. Consensus indicates that there will be continue financial pressure on healthcare due to labor shortages, inflation, and reimbursement contraction. However, we feel technology adoption to maximize workforce challenges and create efficiency, changing payer mix, movement towards non-acute setting like home health will be a focus. Federal government would like more value based care and reimbursement. One example of a critical enabler of focus is the integration of Remote Therapeutic Monitoring (RTM) solutions, such as those powered by Digital Conversation. RTM facilitates better patient engagement and real-time data collection, allowing providers to monitor treatment adherence, manage chronic conditions, and optimize outcomes while reducing costs. Digital Conversation’s solutions simplify RTM adoption, helping payers and providers leverage this technology to improve operational efficiency and unlock new revenue streams, particularly in the growing Medicare and Medicaid duals population and value-based care models. Improved workforce management to include better patient response management at scale. 7. Voice and video recording features. 8. Improved Patient Portal through Hub messaging. Lets all thrive in 2025!

Recently, I was asked an all too re-occurring question by our clients: "What’s the best communication strategy for primary care within a critical access hospital to promote CCM, RPM, and remote therapeutic monitoring effectively to their provider network—and maximize onboarding?" Followed by an equally critical question: "How do we acquire and activate patients in programs like these successfully?" These conversations about both provider engagement and patient activation—two sides of the same coin, each essential for success. Here's what stood out: The most effective strategies start with aligning communication to the provider’s workflow. Providers are inundated with demands on their time and focus. The key is presenting patients who qualify in a way that’s: Data-driven: Leveraging analytics to provide actionable insights. Integrated: Seamlessly incorporating this data into the provider’s existing workflow. If I have not said it clearly, “workflow, workflow, workflowâ€. Client organizations that have done this successfully have seen incredible results. On the flip side, those that struggle often lack the necessary alignment with their network or face hurdles related to perceived authorization gaps. The patient is central to all of this. For a program to succeed, patients must be: Educated: They need clear, relevant, and tailored information. Consented: Informed consent is not just compliance—it's trust. Engaged: Sustained involvement over time requires consistent value delivery. The best patient activation strategies focus on delivering information that is: Relevant: Avoid redundancy, and ensure the content addresses their concerns and questions. Timely: Share insights retrospectively (what happened), concurrently (what’s happening), and prospectively (what’s next).
