The American Diabetes Association has reported that the prevalence of diabetes in the long-term care (LTC) population ranges from 25% to 34%, which disproportionately affects the elderly in the US. Diabetes increases the risk of various geriatric syndromes, such as falls, cognitive impairment, depression, and urinary incontinence. Care and management of diabetes in the LTC setting require individualized strategies, as severe hypoglycemia and persistent hyperglycemia can lead to adverse outcomes, including hospital readmission. Due to the clinical complexity and psychosocial heterogeneity of the older population in LTC facilities, innovative thinking and a personalized approach are essential. Medical providers see patients at least once every 30 days for the first 90 days after admission and at least every 60 days thereafter, but patients may have uncontrolled blood glucose levels without the practitioner's knowledge. Staff and practitioners face challenges such as knowledge deficits, lack of team communication, and inappropriate dosing or timing of insulin, among others. It takes a dedicated interprofessional team composed of registered nurses, certified nursing assistants, diabetes educators, dietitians, food service managers, consultant pharmacists, physical therapists, social workers, and practitioners to provide optimal care for older patients with diabetes in LTC facilities. Individualized care and effective team communication are crucial to ensure quality care and positive outcomes.
