    Joe Paduda’s take on Republican healthcare cuts ignores how the system really works. In a recent article, Paduda predicted that doctors will recoup revenue by: ??"Jacking up prices" for treating injured workers ??Delivering unnecessary care ??Keeping workers out of work longer ??"Hoovering" more money from insurers and other payers. Reality check: doctors don’t control prices, treatment approvals, or claim durations—governments and claims administrators do. ???????? ?????? ?????? ??????????: ? Doctors don't determine reimbursements. Fee schedules dictate reimbursement rates (though payers undercut those rates with PPOs and other discount mechanisms). ? Doctors don’t game the system by inflating services. Treatment guidelines and UR dictate care, and claims administrators can deny any request. ? Doctors don’t decide how long workers stay off the job. Delays come from UR denials, claim disputes, and bureaucratic hurdles. ? Doctors aren’t the ones "hoovering" money from the system.     ?? Callout: Liberty Mutual is making workers’ comp billing harder for doctors. California requires claims administrators to send ???????????????????? ???????????????????????? ???? ???????????? (??-????????) when providers submit e-bills. e-EORs automatically update providers’ billing systems with payment details—saving tons of time. Yet in January alone, Liberty Mutual failed to send e-EORs for 642 out of 5,856 e-bills. That’s an 11% non-compliance rate. This follows a year-long pattern. For each of those bills, the provider has to: ? Figure out whether Liberty Mutual paid at all ? Figure out whether Liberty Mutual paid timely and correctly ? Upload payment information to their billing systems This means wasting ???????????????? ???????? ?????? ?????????????????? that could be devoted to patients. The bill ???????????? goes to the claims admin. ? Insurers and self-insured employers may handle their own claims administration—or they might use a Third-Party Administrator (TPA). ? Knowing which entity is the claims administrator is step one for successful billing. ? The ???????????????? is responsible for making sure their employees have workers’ comp coverage. ? In every state except Texas, employers have to provide or secure workers' comp coverage for their employees. They may self-insure or use an insurer. ? If a self-insured employer also self-administers, the employer is the claims administrator. ? Many self-insured employers, however, hire a TPA to handle claims administration. ? The ?????????????? provides workers’ comp coverage for the employer. ? In many cases, the insurer is the claims administrator. ? In some cases, the insurer hires a TPA to handle claims administration. ? The ?????????????????????????? accepts e-bills on behalf of the claims administrator. ? Most insurers, self-insured employers, and TPAs lack the capability to accept e-bills—even though many state laws require them to. ?
