Whether it's getting connected to a new PCP or affordable housing resources, health workers can continue to provide care navigation to their clients, now through streamlined, AI-powered access to health information and resources. Through this, Culture Care Collective focuses on whole health to bridge gaps in health services and improve the health and social capital of everyday people.
