  • The Affordable Care Act (ACA) has brought critical benefits to Boomers and aging adults. It ensures access to preventive care services like cancer screenings and wellness visits, caps prescription drug costs for Medicare beneficiaries, and offers protections for those with pre-existing conditions. For seniors managing chronic illnesses, these provisions have been a lifeline. But with discussions of repealing or overhauling the ACA, these protections could look very different in the future. While we don’t know exactly how changes to the ACA might unfold, what we do know is that they could fundamentally alter what Boomers have come to understand and rely on in their healthcare. We can only hope these changes will lead to better care, but there’s no certainty—and that uncertainty alone could create confusion for seniors navigating an already complex system. For aging adults, stability in healthcare access is crucial, and any shift at this stage could present significant challenges. Healthcare professionals who play a role in decision-making can take action today. Advocate for solutions that protect seniors' care and ensure equitable access for vulnerable populations. Educating your teams and communities about these potential shifts can help prepare everyone for what lies ahead. Check out this article from the Common Wealth Fund>> https://lnkd.in/gMhq3NWy

  • Who fills in the gap when Medicare falls short? I know no one wants to talk about it, but we have to address the impact of the Inflation Reduction Act on healthcare—and the hard truth that Medicare services are being dropped in some areas across the country. We need to recognize what healthcare is beginning to look like, especially for our aging population. Seniors already face an uphill battle with complex paperwork, overwhelming jargon, and endless mailers. Now, imagine coupling that with the devastating news that the essential care they’ve worked hard for is no longer accessible. So, who fills the gap when Medicare pulls out? Health plans have an opportunity to bridge this gap and support our aging population: ?Provide clear, accessible communication: Streamline information to help seniors understand their options without confusion, delivering critical updates in simple, straightforward language. ?Build community-based support systems: Partner with local organizations to fill service gaps, offering resources that address immediate healthcare needs close to home. ?Prioritize preventive and long-term care solutions: Invest in proactive programs that help seniors manage their health, reducing reliance on emergency or last-minute care. As leaders in healthcare, it’s on us to step forward. Let’s ensure our seniors don’t lose access to the care they deserve. #Medicare #HealthcareAccess Image courtesy of Ai

Community Health Workers embedded within care teams are powerful liaisons between the healthcare system and underserved populations. By training CHWs to navigate patients through complex healthcare systems while addressing non-medical needs (housing, food security), your organization can drastically reduce gaps in care and improve patient trust—proven to decrease hospital readmissions by 30%. 2. ?????????????????? ???????????? ???????????? ?????? ?????? ?????? ???????????????? ?????? ?????????????????????? Before rolling out any new policy, perform a Health Equity Impact Assessment (HEIA). These audits critically evaluate how different patient populations may be affected by a policy, ensuring no group is disproportionately disadvantaged. This method has helped healthcare organizations anticipate unintended consequences and mitigate disparities before they arise. 3.???????????????? ???????????????????? ?????????????????? ???? ???????????????????????? ?????????????? ???????????? ???????????????????? Use data analytics to identify trends in patient populations that point to emerging disparities before they worsen. This allows organizations to preemptively allocate resources to at-risk populations and areas where SDOH are most likely to affect outcomes. Some healthcare systems have reduced emergency room visits by 15% through predictive modeling that targets high-risk communities for early interventions. 4.???????????? ????-???????????? ???????? ???????????????? ???????? ?????????????? ?????????????????????? Invite patients from diverse cultural, racial, and socioeconomic backgrounds to actively participate in the design of care services, pathways, and even physical spaces. Co-designing with patients, rather than designing for them, has been shown to increase patient satisfaction by over 20%, while making care delivery more culturally responsive and inclusive. 5. ???????????? ??????????-???????????? ???????????????????????????? ?????? ???????????????? ???????? ???????????????? Addressing health equity requires partnerships beyond the healthcare system. Establish formal collaborations with sectors like education, housing, and employment services to provide holistic care for patients. By integrating these external services into the patient care continuum, organizations can address the root causes of health disparities. Some hospitals have seen improvements in patient adherence and outcomes by offering onsite housing assistance and job training as part of their care strategy. Is your model ready to meet the challenges of health equity? Let’s ensure your strategies are up to the task.

  • Patients Want to Know… 1. ???????? ?????? ???????????? ???????? ???? ?????????????? ????????? Studies show that when patients feel their concerns are not heard, trust erodes, leading to worse health outcomes. Over 60% of patients who don't feel listened to are less likely to follow critical health advice, impacting their overall well-being. 2. ?????? ???????? ?????? ?????????????? ?????? ???????????????????? ???????? ??????’?? ??????????????? Social determinants like financial instability and lack of transportation affect up to 80% of health outcomes, yet nearly 90% of physicians report they lack the time and resources to address these factors adequately. Ignoring these realities can lead to increased disparities in care. 3. ?????? ???????????? ?????????????????????? ?????? ???????? ???????? ??????????????? Implicit bias still drives unequal treatment. Black patients, for example, report feeling dismissed by doctors at higher rates than their white counterparts. It's time for us to challenge these biases and ensure every patient feels believed and respected. 4. ???????? ???????? ???????????????? ???? ???????? ???????????? ??????????? Trust is the foundation of effective healthcare. Patients who feel dismissed are 30% less likely to adhere to treatments. Building partnerships with patients—by listening, engaging, and acknowledging their unique challenges—improves both patient satisfaction and outcomes. 5. ?????? ???????? ???????????????? ???????? ?? ????????????????????? Almost 95% of doctors agree that addressing social factors could improve patient health, yet only 19% feel they have the resources to do so. • Women's Equality Day

    查看Jay P.的档案

As we celebrate 104 years of women's equality/voting rights, it’s important to remember that it took 144 years from the founding of the United States for women to secure the right to vote. However, this milestone wasn’t the end of the struggle for equal voting rights. It took another 45 years until the Voting Rights Act of 1965 ensured that Black Americans could fully exercise their right to vote, free from brutal obstacles like literacy tests. Just 60 years ago this month, Fannie Lou Hamer delivered her powerful testimony at the Democratic National Convention, recounting the violence she faced in her fight for voting rights. Her words remind us that the journey to equality has been long and hard-fought. Today, let’s honor the resilience of those who paved the way for all women to vote. #womensequalityday #August26 #votingrights
