Our Mission The mission of the Care Transformation Collaborative is to lead the transformation of primary care in Rhode Island in the context of an integrated health care system; and to improve the quality of care, the patient experience of care, the affordability of care, and the health of the populations we serve. To engage providers, payers, patients, purchases and policy makers to develop high quality, family and patient centered medical homes providing health care in an affordable, integrated healthcare system that promotes active participation, wellness and delivers high quality comprehensive health care dedicated to data-driven system reform. Our Vision Rhode Islanders enjoy excellent health and quality of life. They are active participants in an affordable, integrated healthcare system that promotes wellness and delivers high quality comprehensive primary care. The Care Transformation Collaborative of Rhode Island has a growing impact across the state, and includes: ? 124 primary practices, including internal medicine, family medicine, pediatric practices and 10 primary care practices are part of integrated behavioral health initiative. ? Approximately 695,000 Rhode Islanders receive their care from one of our practices. ? 800 providers across our adult and pediatric practices. ? Investment from every health insurance plan in Rhode Island, including private and public plans. ? All Federally Qualified Health Centers in Rhode Island participate in our Collaborative. ? CTC practices demonstrate lower total cost of care by $217 million in 2016 when compared to non-patient centered medical homes in Rhode Island, according to data from the state’s All-Payer Claims Database.

Proud to be part of the RI contingent to #APHA24 in Minneapolis to talk about all the advanced CHW work we are leading. For a small state, we are ahead of where many states are with our standing orders, Medicaid reimbursement, and incredible CHW leaders. But we still have lots of work ahead to ensure sustainability and integration across all settings.

At CTC-RI, our primary care transformation efforts are bolstered by partnerships with incredible community-based organizations (CBOs) across the state. Each month, we spotlight a different CBO to recognize their impactful work. This month, we’re proud to highlight the Parent Support Network of Rhode Island (PSNRI), a family and peer-run nonprofit that has been serving the behavioral health community since 1993. What does PSNRI offer? PSNRI provides community health services, peer recovery, and support for accessing healthcare, housing, trauma treatment, education, and employment. With a team of over 50 dual-certified community health worker-peer recovery specialists, PSNRI works with children, youth, adults, and their families to create personalized care plans that address their unique strengths and needs. How can primary care providers partner with PSNRI? PSNRI is ready to expand partnerships with primary care providers across Rhode Island to make healthcare more accessible for children, families, and individuals in need. Together, we can offer home and community-based health education, care coordination, and peer support for a variety of populations, including pregnant women, children in behavioral health treatment, and those facing housing crises. ?? Kickoff: Delivering Age-Friendly Care Learning Collaborative! In September, CTC-RI successfully launched the “Delivering Age-Friendly Care” learning collaborative, aimed at helping practices implement the 4M Model and improve care for older adults and individuals living with dementia. Over the next six months, six participating practices will embark on a quality improvement journey, working toward Level 1 or Level 2 Recognition from the Institute for Healthcare Improvement (IHI). In partnership with RIDOH and the RI Geriatric Education Center (RIGEC), this initiative also focuses on supporting care partners of patients with dementia by ensuring they receive education and proper referrals. ?? CTC-RI's Best Practices in Team-Based Care Series Continues! ?? In September, we hosted a dynamic session on “Improved Communication between School Nurses and Pediatrics for Better Management of Students' Medical Conditions” as part of our Best Practices in Team-Based Care series. This session highlighted practical tools to enhance collaboration between pediatric practices and school nurses, focusing on managing chronic conditions like asthma to improve student care and reduce disruptions to learning. We were thrilled to have Dr. Maureen Crotty (Blackstone Valley Pediatrics), Dr. John Butler (Barrington Pediatric Associates), and a group of dedicated school nurses including Bethany Goldberg, Tracey Bradley, Colleen O'Donnell, Erika Iafrate, and June Tourangeau, share their expertise. Together, they explored strategies to strengthen communication, improve care coordination, and better support students with chronic conditions. ??
