Meet Priya Byati, CRISP DC's Program Coordinator! In her role, Priya is responsible for: ? Managing the DOEE lead registry project ? Advancing our care planning program ? Managing the DC CARE grant initiative and the ONAF form initiative ? Coordinating integration between the outreach and technical team for various organizations ? Communicating meeting minutes for various external and internal meetings ? Last Thursday, Priya Byati represented CRISP DC at the DC American Academy of Pediatrics (AAP) Fall Symposium, which focused on addressing school absenteeism—a critical issue impacting the health and well-being of students across the District. The symposium featured a powerful keynote and panel discussion by city and pediatric leaders who are dedicated to improving attendance and supporting our community's youth. Priya had the opportunity to connect with many inspiring professionals, including the amazing Children’s CARE-H team (check out our group photo below). It was an insightful evening filled with valuable conversations about how we can work together to better serve our students and their families. Our team had an incredible time at the #Civitas2024 Annual Conference this week! A special shoutout to FRANCESCA CHARLES, MHI, M.Ed., Deniz Soyer, Richard Garcia, MA, CAPM, and Donna Ramos-Johnson for rocking the panel on DC’s unique approach to population health management! The panel highlighted CRISP DC’s HIE designation and the trusted partnerships we’ve built across the district. Francesca shared the impactful work we've been doing integrating data automation and advanced population health analytics, including our collaboration with hMetrix. These efforts have enabled District providers to improve patient care and drive better health outcomes. It was an honor to showcase how CRISP DC’s data-driven initiatives continue to enhance care delivery in Washington, DC.
