Community Care Cooperative (C3) is a 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by Federally Qualified Health Centers (FQHCs) in Massachusetts. In 2016, leaders from 15 FQHCs throughout the state formed C3. An innovative model for an Accountable Care Organization (ACO), C3 leverages the proven best practices of ACOs throughout the country. C3 is the only ACO in the state founded and governed by FQHCs exclusively focused on advancing integrated and coordinated community-based care for MassHealth members. C3 is building its organization on the collective strengths of health centers. We currently have 17 partner health centers across the state and our continued growth and expansion enables C3 to better serve MassHealth members across the Commonwealth.

On Dec. 3rd for our 4th Annual "Diversity, Equity, and Racial Justice Symposium," where we will explore "Gender Affirming Care: Importance, Impact, and Implementation," with our special guest Julie Thompson from Fenway Health! Julie will show us how social stigma and discrimination impact access to care, health, and well-being, and how we can define and explore medical gender affirmation. We will also look at what are some of our options and considerations. In addition, we will also look at the diversity of sex, gender identities and sexual orientation, and discuss how to take a trauma-informed, affirming, and anatomy-based medical history. Following Julie's presentation, we will hear from Steph deNormand, Genesis Valera, and Justice Williams, three gender diverse individuals who will share their experiences engaging and navigating the health care ecosystem. They will discuss the challenges and barriers and share some successes and suggestions for creating inclusive and safe spaces within healthcare. What is some of the information we happily share with Federally Qualified Health Center leaders looking to join our national Accountable Care Organization? 1. We are the largest ACO in Massachusetts, and we contract in seven states and the District of Columbia. (For now. The "seven" will be expanding, dramatically, in 2025.) 2. We have re-invested more than $64 million in value-based savings to support community-based primary care with our partners. 3. Community health organizations such as Fenway Health are among those leading the charge towards better health outcomes for LGBTQ+ patients. Its Trans Health Program provides patient-centered, evidence-based care to ensure trans and gender-diverse people are informed and affirmed in their gender and healthcare needs. The organization advocates for policies at the local, state and federal levels that protect and affirm all members of the LGBTQIA+ community, including TGD people. An intersectional and responsive approach to care, one that acknowledges the intersections of identity and how they contribute to communities’ unique health needs, is key to advancing health equity. The theme for today's World COPD Day is "Know Your Lung Function." All of us in health care, and particularly those of us who work for, and support, Federally Qualified Health Centers, are focused on highlighting the importance of measuring lung function, also known as spirometry. While spirometry is an integral tool for diagnosing COPD, it is also used to measure lung health throughout life. In order to treat the disease in patients throughout the country, many Federally Qualified Health Centers have implemented elements of the COPD National Action Plan into their care plans. This comprehensive framework focuses on COPD prevention strategies such as smoking cessation and smoke free policies; early-diagnosis treatment that relies on medication and oxygen therapy; and the provision of access to pulmonary rehabilitation and caregiving, which is designed to prevent exacerbations. All of those measures, and a plethora of other treatments, are designed to slow the progression of the disease and reduce symptoms. #WorldCOPDDay #COPD #NationalCOPDDay #Spirometry #Healthequity #CommunityHealth #CommunityHealthCenters #HealthyLungs #LungFunction

I usually don’t mix my personal life with my professional life, however since we’re in a month where we celebrate gratitude, I’d like to take a moment to thank each and every person has helped me to grow into the person I am today, both professionally and as a person. I was a young mother and high school drop out working multiple jobs just to make ends meet while going through domestic violence with my children. Thanks to organizations like REACH Beyond Domestic Violence and Found in Translation, Inc. I am able to call myself a survivor and be proud of taking the cards I was handed and turning them into something positive where I could help people. With all of the events occured and organizations involved eventually leading me to Community Care Cooperative (C3) where as a team we are able to advocate for our members, many of whom have passed through or are going through very difficult situations, just like I did, and trying to navigate the system on their own! Just a very important reminder that life is only 10% what happens to you, and 90% how you react! Had you told me 10 years ago when I was 22 and still had no had any interaction with those who helped me that I’d be a supervising the call center for the largest Masshealth ACO I would not have believed you, but here we are today!
