Community Care Cooperative (C3) is a 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by Federally Qualified Health Centers (FQHCs) in Massachusetts. In 2016, leaders from 15 FQHCs throughout the state formed C3. An innovative model for an Accountable Care Organization (ACO), C3 leverages the proven best practices of ACOs throughout the country. C3 is the only ACO in the state founded and governed by FQHCs exclusively focused on advancing integrated and coordinated community-based care for MassHealth members. C3 is building its organization on the collective strengths of health centers. We currently have 17 partner health centers across the state and our continued growth and expansion enables C3 to better serve MassHealth members across the Commonwealth.

  • We are excited to announce that Annie Pham, MHSA and Kim Prendergast will be part of two presentations at "The Rise Summit on the Social Determinants of Health" April 7th-9th in Louisville, KY! In the first presentation, Annie will talk about the "Social Health Hub" model C3 has built with its community partners to deliver #foodismedicine and housing interventions to patients throughout our health centers. She will be joined by Camden Coalition's Mark Humowiecki and Sarah Stacy from Uncommon Solutions, to discuss what a national approach to this work would look like, and how community care hubs can support Medicaid Section 1115 initiatives. The trio will also compare our model in Massachusetts with what is being done in Washington state. The second presentation will feature Annie and Kim joined by Jennifer Obadia from Project Bread and delve into the organizations' five-year partnership, that has built a robust program that delivers medically-tailored nutrition services to individuals experiencing diet sensitive and chronic conditions. As the only Accountable Care Organization (ACO) in Massachusetts founded and governed by Federally Qualified Health Centers (FQHCs), we are thrilled to announce our selection by Centers for Medicare & Medicaid Services (CMS) to participate in the Medicare Shared Savings Program (MSSP) Enhanced Track and the ACO Primary Care (PC) Flex Model! Community Care Cooperative (C3) is one of 24 ACOs participating in the ACO PC Flex model in performance year 2025. C3 shares in the risk and reward across 43 FQHCs’ value-based agreements, representing more than 45,000 Medicare beneficiaries. C3's recently awarded MSSP Enhanced and ACO PC Flex participation agreements extend C3’s network to approximately 6,000 Medicare beneficiaries, enabling us to further our mission of uniting FQHCs at scale to strengthen primary care and improve the health and wellness of the people we serve. FQHCs are known for delivering high-quality, person-centered care that reduces total medical expenditures. C3 aims to support their success in improving outcomes by incentivizing participating health centers to maintain lofty standards of care while managing costs. C3 is excited to welcome five participating FQHCs from Massachusetts and one from Oregon to its CMS MSSP Enhanced Track and ACO PC Flex participation agreements. These strategic partnerships underscore C3's unwavering commitment to empowering healthcare providers on their journey to value-based care. The new participating FQHCs are: Community Health Center of Cape Cod, MA Holyoke Health Center, MA Island Health Care, MA Lynn Community Health Center, MA Mosaic Community Health, OR North Shore Community Health, MA “We are enthusiastic about the opportunities the new Medicare participation agreements present and remain steadfast in our mission to deliver accessible, comprehensive, and compassionate care nationwide,” said Christina Severin, CEO of C3. “The MSSP and ACO PC Flex model create a robust framework for expanding access to high-quality primary care and addressing health disparities, particularly in underserved communities. FQHCs are an integral part of the healthcare system, and it is an honor to serve as an ACO partner for our participating FQHCs.” C3’s growing Medicare ACOs include approximately 1,500 primary care physicians and over 3,200 clinicians operating across communities in the District of Columbia, California, Connecticut, Louisiana, Massachusetts, North Carolina, Oregon, and Washington. C3 represents over 240,000 Medicaid and Medicare beneficiaries in a value-based risk arrangement and has earned over $84 million as an ACO since 2018. ?
