Live Your Best Life

关于我们

Community Living Connections, Inc. is a local, non-profit agency founded in 2004. CLC provides home-based and community-based supported living services to approximately 250 individuals who live throughout the communities of Madison, Beaver Dam, Fond du Lac, Platteville, and Stoughton - Wisconsin! We respect each person's unique contribution to their community. We ensure person-centered supports that emphasize dignity, respect and independence. Our services aim to connect individuals with their community and help them reach their full potential. The success of our organization is measured by the individual successes of the people we serve. Adults with developmental disabilities living their dreams, traveling, buying their own homes, getting married, working, owning a business - choosing and directing their own lives. Successes that are helped made possible by the dedication and commitment of the people who work here.

?? Thankful for Our Incredible CLC Community ?? As we gather this Thanksgiving, we’re filled with gratitude for the heart and dedication of our amazing staff and program participants who make CLC so special. ?? Donna, the mom of one of our participants, recently shared this touching message about Joy, one of our staff members: "Joy has been so wonderful in assisting Austin. She’s so on top of things and at times thinks of things I’ve forgotten. Austin's life runs so smoothly, and I never have to worry about his care with her. It’s during this time of Thanksgiving that she comes to mind. She has truly improved Austin's quality of life. A HUGE thank you to Joy and Austin’s CLC team!" This message is a beautiful reminder of how our work creates peace of mind and transforms lives. ?? To our staff: thank you for your tireless compassion, problem-solving, and care. To our program participants and their families: thank you for trusting us to walk this journey with you. We are grateful for each and every one of you. Together, we are stronger, more compassionate, and more connected. Happy Thanksgiving from the entire CLC team! ???? #Thanksgiving #Gratitude #ThankYouTeamCLC #BuildingCommunity Image Description: Thanksgiving graphic with festive images of CLC program participants
