By providing seamless access to a variety of healthcare services, we create healthy communities, one patient at a time.
关于我们
Community Care Physicians (CCP) is the largest independent multispecialty medical group in the Capital Region of NY. Headquartered in Latham, we provide a broad range of healthcare services to 8 counties in upstate NY. Our organization includes primary care, specialty care, urgent care and wellness services. With more than 1,900 employees, including over 421 practitioners in its sub-specialty and primary care network, CCP touches every three to four people in the Capital Region. We are focused on relationships with our patients and teams. We have "community" in our name for a reason. It's something we live and breathe every day. We live here, work here, and our focus is on making our community a healthier place. CCP's achievement as a high performing organization has evolved from a long tradition of excellence. Founded over 35 years ago, what started as a radiology practice has since grown to a multispecialty organization with over 30 medical specialties and some of the area's most highly respected practitioners. For many of us, taking care of people isn't just the career we have today, it's the passion we've had our entire lives. From our greeters to our nurses, practitioners to our support staff, we all are focused on the health – and happiness – of our patients. Our unique business model sets us apart – we have centralized technical and administrative services so our practices can focus on patient care. As a physician owned and managed organization, we are guided by those who practice in our offices every day and focused on creating an optimal experience for our patients and our staff. CCP is a multiple recipient of the Albany Times Union's "Top Workplace," the Albany Business Review's "Great Place to Work," in 2021, Energage ranked CCP as a "Top Workplace USA," and have been recognized by our patients for our high-quality care. Come drive quality, inspire compassion, and contribute to our community, one patient at a time. Bring your talents to CCP.

? Self-Care Tip! ? Take a break from your busy day and walk outside to clear your mind. Fitting in a daily 30-minute walk, especially outdoors, has powerful effects on your brain functioning. This includes improved attention span, memory, executive functioning, and focus. It also reduces mental fatigue and tiredness. Taking a stroll outdoors during your lunch break can make the rest of your workday much more productive. #WorldMentalHealthDay #WMHD2024

Avoid exercise within a few hours before bed. ??Avoid naps or limit to only one 30-minute nap a day in the early afternoon. ??Set aside worry/planning time (15-20 min) early in the day to sit quietly and think about what you need to do or are worried about. Avoid worrying at bedtime. ??Take a walk in the afternoon to increase light exposure and increase evening alertness. PRIOR TO BED ??Avoid evening caffeine, nicotine, or alcohol. ??Do something relaxing just before bed – meditate, listen to relaxing music, and read. ??Avoid stimulating activity just before bedtime. ??Eat a light bedtime snack – avoid going to bed hungry. ??Limit liquids if you have a problem with getting up during the night. BED ACTIVITY ??Only sleep in bed! Avoid reading in bed, watching TV in bed, eating in bed, etc. Avoid TRYING to fall asleep. If you cannot fall asleep within 15-20 minutes, get up and do something relaxing in a dimly lit room until you feel your eyes closing, and then go to bed. If still unable to fall asleep, get up again and repeat until able to fall asleep within 15-20 minutes of getting into bed. ??Don’t look at the clock when you wake up in the middle of the night or can’t fall asleep. Turn the clock around. ??If you wake up in the middle of the night, don’t turn on the overhead light. If necessary, use a night light to go to the bathroom.
