Every year, several of our HOP HSOs delivering food boxes provide an extra box to families filled with holiday goods. This year is no different. A special thank you to Cape Fear HOP HSOs who are always busier during the holidays serving those in need. We are grateful for the work you do to lift up communities. 关于我们
At Community Care of the Lower Cape Fear (CCLCF), we are changing the healthcare experience by changing the way care is delivered. We bring over 18 years of experience providing locally-based care management with a proven strategy for health care quality improvement and cost reduction to southeastern North Carolina. We work with Prepaid Health Plans, ACOs, practices and providers to improve patient outcomes, experience, and satisfaction. CCLCF is an independent, 501(c)(3) nonprofit and a National Committee for Quality Assurance (NCQA) accredited organization. CCLCF was launched in 2003 as an independent 501(c)(3).
