This week we presented two key CCB initiatives at the Maimonides Health evening of research! ??the Community Care of Brooklyn-New York Legal Assistance Group Legalhealth Clinic is charting a path for clinical-legal partnership and offers data on the most common legal needs of referred patients. ??our CCB Navigator program is growing! Referral numbers increased in 2024 and offer insight into the resources and services Brooklyn residents need most. Congratulations to our presenters Magdalena Gordon, LMSW and Dylan Jew!

We've teamed up with Community Care of Brooklyn through our Unite Us Payments solution to reimburse community-based organizations at scale for the important services they provide. With Unite Us Payments, funders are able to track grant funding, approve client eligibility, and authorize the amount or duration of a service that an individual is qualified to receive. We’re excited to announce the launch of the CCB Navigator Payments Pilot, an innovative program that adds payment and tracking capabilities to the CCB Navigator. Through the pilot, we’re creating a model for funding the crucial assistance Brooklynites receive from community-based organizations. We've partnered with Housing Works, Family Services Network of New York, and Fifth Avenue Committee to help qualifying individuals experiencing food insecurity, inadequate or unstable housing, and/or difficulties navigating social services. ? Big news today for health equity in NYC! This morning CMS approved a new 1115 Medicaid waiver amendment. We are very excited about this investment in addressing health-related social needs and health disparities across the state. Some key inclusions in the waiver are: ??Establishing Social Care Networks to integrate health, behavioral, and social care services ?? Investing in primary care ???? Funding for health care workforce programs ?? Advancing Value-Based Payment (VBP) strategies ??
