As the Centers for Medicare & Medicaid Services’ QIN-QIO 12th Scope of Work has ended, Comagine Health is celebrating by sharing some of the program’s impact over the past five years.?? From 2022 to 2024, Comagine Health expanded access to free naloxone in four rural, underserved communities across Washington, Nevada and Idaho. By building partnerships with local public health departments and developing patient-focused collateral for pharmacists to distribute, we helped connect patients with lifesaving medication.? Collateral for pharmacies included “naloxone saves lives” bag stickers; cards with information on how to access free naloxone; and tear-off pads that allowed patients to discreetly request a conversation with the pharmacist about naloxone.? Comagine Health distributed 4,800 bag stickers, 50 tear-off pads and 750 naloxone referral cards to partner pharmacies. Pharmacists placed at least 1,053 stickers on bags, were handed 68 naloxone discussion request sheets and shared 115 referral cards with patients at the pharmacy counter. Comagine Health staff also facilitated distribution of 239 nasal naloxone units to partner pharmacies through local public health departments.? As the Centers for Medicare & Medicaid Services’ QIN-QIO 12th Scope of Work has ended, Comagine Health is celebrating by sharing some of the program’s impact over the past five years.? Health equity was a focus across all interventions during the 12th Scope of Work. Through hard work and collaboration across teams, we expanded provider and practitioner knowledge of health equity best practices. Health Equity Learning Events: Comagine Health offered learning opportunities for providers from various care settings. Sessions included topics such as health literacy, implicit bias, vaccine equity, social determinants of health, national culturally and linguistically appropriate services (CLAS) standards and more. Health care professionals attended these events more than 260 times. CLAS Standards Articles: We created a four-part newsletter series on national CLAS standards for providers across care settings. Newsletters reached health care professionals nearly 500 times. Articles covered CLAS fundamentals, providing CLAS to patients, creating a CLAS implementation action plan and resources to provide and implement CLAS. Accessible Collateral: Comagine Health ensured that patient-facing materials were offered in languages that met the individual needs of each community. Our partnership with a Nevada long-term care facility offered materials on preventing catheter-associated urinary tract infections (CAUTIs). We developed materials in English, Swahili, Tagalog and Spanish, ensuring they could be read by all residents. No CAUTIs have been reported at the facility since these materials were distributed. As Comagine Health wraps up the final weeks of the Centers for Medicare & Medicaid Services’ QIN-QIO 12th Scope of Work, we are celebrating by sharing some of the program’s impact over the past five years. When a long-term care facility in Utah was struggling to improve vaccination rates, Comagine Health’s long-term and post-acute care (LTPAC) team worked with them to dig further and reveal the major barrier: the amount time staff members spent preparing and managing vaccine permission and declination forms from power of attorney representatives. The team collaborated with the facility to implement an e-signature program to replace the laborious and lengthy process of disseminating hardcopy mailers, which consumed valuable staff hours and kept them off the floor. After securing facility buy-in, the LTPAC team: * Created a policy and procedure for the e-signature program * Consulted on the design of an e-signature consent form * Created an application training for staff use * Co-designed the pilot and coached on sustainability This reduced the time spent on this task from three hours to just 30 minutes, giving staff members more time to focus on resident care.
