EviCore Turns Up the Dial on Denials: When Profits Drive Healthcare Decisions ProPublica continues to do some of the most important reporting of our time. A new investigation reveals how some companies are using AI to turn denials into a profitable business model. EviCore, which manages care decisions for 100 million Americans, claims cost-cutting is "a happy coincidence." The evidence suggests otherwise. The Denial Dial EviCore uses an internal proprietary algorithm called "the dial" that can be adjusted to increase denials. While marketed as a tool to streamline approvals, former employees reveal: - The algorithm scores each request's likelihood of approval - Management can adjust thresholds to force more manual reviews - More reviews lead to more denials - Sales teams boast of 15% increases in denial rates The Human Cost Patients across the country are being denied critical treatments that their doctors deem necessary. One heart-wrenching case involved a 61-year-old welder whose doctor urgently recommended a heart catheterization, only for EviCore to deny the procedure in favor of a cheaper alternative. Within 36 hours, he suffered a fatal cardiac arrest, leaving behind a grieving family. This tragedy is just one example of how denial-for-profit models are costing lives. By the Numbers - EviCore promises insurers a 3-to-1 return on investment - Nearly 20% of requests are denied in Arkansas, compared to 7% for Medicare Advantage - Doctors must handle 15 cases per hour—one every 4 minutes - "Sentinel effect" celebrated as doctors stop requesting care This isn't just EviCore. Companies like Carelon Medical Benefits Management have built an entire industry around denial-for-dollars. When algorithms prioritize cost savings over patient care, the mission of healthcare is lost. Earlier this week, I shared how Massachusetts hospitals crumbled under financial pressure, with Steward Healthcare's bankruptcy forcing two facilities to close. This wasn't an isolated incident. Hospital closures are sweeping across America at an alarming rate. In August 2024 alone, five healthcare facilities shut their doors permanently: ?? The latest closures include: 1. Massachusetts: Nashoba Valley Medical Center and Carney Hospital closed simultaneously, leaving 1,243 healthcare workers jobless. 2. California: Stanislaus Surgical Hospital suspended operations, laying off 160 employees. 3. Iowa: MercyOne announced multiple facility closures, including the Primghar Medical Center. 4. Ohio: Community Memorial Hospital shut down, leaving a rural community without local care. 5. Texas: The Hospital at Westlake Medical Center closed its ER after filing for Chapter 11 bankruptcy. ?? A devastating pattern is emerging. From urban hospitals filing for bankruptcy to rural communities losing their only healthcare provider, these closures are creating healthcare deserts nationwide. ?? “The decision to close Community Memorial Hospital follows thorough efforts to find a viable path to continue operations," stated former CEO Bill Cherry. "Unfortunately, despite all our efforts, CMH could not find a sustainable solution.” These closures aren’t isolated incidents—they’re the result of mounting financial pressures, regulatory hurdles, and declining margins, pushing facilities to their breaking points. ?? Policy decisions have a real human cost. Texas’ aggressive Medicaid “unwinding” has left millions of Texans without healthcare coverage. A crisis is unfolding in the Lone Star State. During the pandemic, Texas' Medicaid rolls grew by over 50% to 6 million people. When federal protections ended in April 2023, states had to reassess eligibility. Texas moved swiftly and reviewed 4.6 million cases in just 6 months — far exceeding federal guidelines. What was the result? Over 2 million Texans lost coverage, including many who are still eligible. Why does this matter? 1. Texas already insures a smaller percentage of its citizens through Medicaid than almost any other state. 2. 1.4 million of those who lost coverage lost it due to a broken bureaucracy and not ineligibility. 3. The federal limit for processing applications is 45 days, while Texas’ median is nearly 3 months. The human impact has been devastating. Children have been forced to forgo lifesaving operations, and a boy with a rare heart condition has lost access to a $6,000 per month medication. The outcomes have been tragic and families throughout the state are facing crushing medical debt. "If we have children who are less healthy, who are unable to get the preventative care they need for their chronic medical conditions, that fundamentally should raise concern for all of us.” - Dr. Kimberly Avila Edwards, a Texas pediatrician. The federal government has launched an investigation, but solutions are complex.
