??AI improves accuracy of screening for #CervicalPrecancer Artificial intelligence (AI) has the potential to transform health care delivery, including human resource challenges in low-and middle-income countries (LMICs). One promising use case for AI is in the detection of cervical precancer. If precancer is caught early, it can be treated – preventing the development of cervical cancer, which is among the leading causes of cancer deaths in women in LMICs. In most LMICs, women are screened for precancer through a naked-eye visual check of the cervix, known as visual inspection with acetic acid (VIA). VIA is simple, feasible, and inexpensive, but highly subjective and known to have poor diagnostic accuracy. To improve the accuracy of VIA-based screening, GH Labs and CHAI (with Unitaid support) have developed an AI-based tool known as Automated Visual Evaluation (AVE). AVE runs on a smartphone and analyzes a nurse’s photo of the patient’s cervix in real-time – without requiring internet access—to determine whether precancer is likely present or absent. We evaluated AVE’s performance in a study involving over 25,000 women across five countries: Malawi, Rwanda, Senegal, Zambia, and Zimbabwe. AVE-assisted VIA detected 71.8% of precancerous cases, compared to just 36.6% detected by traditional naked-eye VIA. The difference in sensitivity was highly statistically significant. AVE also demonstrated strong performance among women living with HIV and those testing positive for HPV, highlighting its effectiveness across diverse populations and its suitability for various use cases, including both primary screening and triage. Earlier this year, we shared preliminary results at national dissemination events in the countries where the study was conducted. Stakeholders, including Ministry of Health officials and clinicians, expressed strong enthusiasm for AVE as a true "point-of-care" test that enables both screening and treatment in a single appointment—an essential feature of convenient, women-centered care. “The current method, VIAC, is resulting in missed cases. Clients, WLHIV and the general population, are showing up at treatment points with advanced invasive cervical cancer while they have been in screening programs. There is need to explore screening methods with higher sensitivity. This study has cemented observations we have been seeing on the ground. The AVE-assisted VIA offers a better solution, and the country should explore its use as quickly as is practical rather than continue with VIAC.” - Dr Grant Murewanhema, Gynaecologist, University of Zimbabwe Medical School Results from the study are being formally presented this week at the International Papillomavirus Conference (IPVC) in Edinburgh, UK. ??With CHAI support, Uganda’s Ministry of Health has launched the revised National Implementation Plan for Medical Oxygen Scale-Up (2023/24 – 2027/28). CHAI supported the evaluation and review of the previous strategy, including comprehensive desk reviews and assessments across 85 health facilities. Based on the findings and insights gained from both the #COVID19 response and ongoing programmatic interventions, we helped draft the plan. The plan followed an approval pathway starting with presentations to the MOH Clinical Care and Infrastructure Technical Working Group. It received endorsement from the relevant senior-level committees before being signed off by the Minister of Health and officially launched at the 4th Annual National Safe Motherhood Conference in October 2024. The updated strategy addresses critical gaps in medical oxygen access that lead to preventable deaths among patients with hypoxemia. Key challenges identified during the evaluation include: ?Limited provider capacity to diagnose and manage hypoxemia (only 8 percent of staff had training). ?Insufficient equipment availability (65 percent of facilities had the recommended number of cylinders, only 51 percent had concentrators, and 23 percent had pulse oximeters). ?Suboptimal oxygen supply systems (only 58 percent of oxygen plants were functional). ?Inadequate data management for monitoring hypoxemia. ?Insufficient funding, with only 51 percent of facilities budgeting for medical oxygen services. The national strategy aims to reduce mortality and morbidity from hypoxemia by addressing these gaps and improving oxygen access nationwide. Over the next five years, it will focus on creating an enabling environment for hypoxemia management, expanding high-quality diagnostic and oxygen supply systems, enhancing clinical use and equipment maintenance, strengthening coordination mechanisms, securing sustainable financing, and improving data collection and usage for hypoxemia management. #WorldPneumoniaDay2024 #InvestInOxygen

Every day, at least one child dies every 45 seconds from #pneumonia, that is over 700,000 children under five, including over 150k newborns per year. Pneumonia can lower blood oxygen, leading to organ damage and failure, and even death. At least 35% of deaths from childhood pneumonia could be averted with medical oxygen. Supplemental medical oxygen increases the amount of blood oxygen taken through the lungs, alleviating symptoms and improving organ function. CHAI is working with Unitaid, Bill & Melinda Gates Foundation, and USAID (as a sub-grantee of FHI 360 on the EpiC Project), alongside partner governments to strengthen oxygen infrastructure and systems in low-and middle-income countries, ensuring that everyone who needs oxygen for treatment has access when they need it most.
