In Our Clinics
What we mean by healthcare at CHN is not your father’s healthcare. We're more than a provider in an exam room. We integrate our care, connecting the many dots to bring comprehensive care to patients. That means there are many additional pieces to what CHN considers basic healthcare. Behavioral health, nutrition, wellness, care management, and ongoing outreach to help patients address the economic and social factors that are the key drivers of good health outcomes. That is healthcare CHN style!
In Our Communities
As a network inspired by diversity, we are committed to serving and celebrating the communities that make New York City unique and remarkable. We recognize the marginalization and the challenges that these communities experience and are devoted to addressing these issues by providing programs – our Transgender Family, Teens P.A.C.T., Military Health, and East New York Health Hub Food Pantry, programs to name a few – and events specific to the needs of these communities.

CHN is rapidly expanding our care capacity beyond traditional primary health services. In 2023, we launched the Jamaica Health Center at the Tree of Life community facility, providing affordable housing. Collaborating with Breaking Ground, we are constructing our Sutphin Blvd. Health Center, next to Sutphin Senior Residence, an affordable housing development for low-income Seniors. Additionally, our Tremont Health Center will soon be located beneath Community Access’ affordable housing.
