The House Energy and Commerce Committee has just advanced HR 1406, the Sustainable Cardiopulmonary Rehabilitation Services in the Home Act, as part of the Telehealth Modernization Act! This game-changing bill will extend virtual cardiac and pulmonary rehab services through 2026, ensuring that even more patients can receive the care they need right from home. Huge thanks to all of our amazing advocates for pushing this forward! But we’re not done yet — now, the bill moves to the full House and Senate for approval, and we need your help to get it over the finish line.     In March 2020, everything changed… COVID reared its ugly head and dramatically reduced non-essential outpatient services including cardiac rehabilitation (CR), many shutting down completely with an incomplete recovery. In fact, one study found that 220 CR facilities closed between 2019 and 2021. As you can imagine, the pandemic made our access, capacity, and patient barrier problems even worse. Now more recent estimates suggest that our already low participation rates are even lower today given disruptions caused by the pandemic.     Prior to the blow of the COVID-19 pandemic, we had made some great strides to address the issue of low participation rates in cardiac rehabilitation (CR). Here's a high-level recap! Million Hearts, a national initiative led by the Centers for Disease Control and Prevention (CDC) and the Centers for Medicare & Medicaid Services (CMS), was launched in 2012 to prevent one million acute cardiovascular events over five years by addressing key cardiovascular disease (CVD) risk factors. Recognizing the importance of CR for secondary prevention, the Million Hearts Cardiac Rehabilitation Leadership Summit was held in November 2015. Strategies for increasing CR referrals and participation were discussed, and ultimately, it led to the adoption of the 70% participation target that was set for Million Hearts 2022. This goal has driven many of the subsequent initiatives. In collaboration with the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR), Million Hearts developed the Cardiac Rehabilitation Change Package (CRCP) in 2018. This tool provides evidence-based strategies for hospitals and CR professionals to improve referral, enrollment, participation, and adherence rates in CR. Additionally, the National Institutes of Health (NIH) funded four projects to boost CR participation, including home-based and hybrid CR models. Results from these trials are forthcoming. In 2018, the Agency for Healthcare Research and Quality (AHRQ) launched the “TAKEheart” initiative, incorporating two Million Hearts-recommended strategies: automatic, opt-out electronic health record referrals and care coordinators to educate and facilitate CR referrals. Also in 2018, the American College of Cardiology/American Heart Association Task Force on Performance Measures introduced new CR performance measures. Subsequently, Million Hearts collaborated with the National Committee on Quality Assurance to develop CR enrollment measures for the 2020 and 2021 Healthcare Effectiveness Data and Information Set (HEDIS).     To add to the complexity of program capacity in cardiac rehabilitation (CR), even the existing number of programs would be insufficient to serve all eligible people. This quote here comes from Dr. Randall Thomas of Mayo Clinic, “If we filled every cardiac rehabilitation program in the United States to full capacity, plus 10%, we could only serve about 45% of eligible patients. We’d need to have a 1-year waiting list.” Think about the implications of a one year waiting list for a moment. The data suggests that for every day a person waits to start CR, that person is 1% less likely to enroll.     Outside of systematic barriers, patients face a vast amount of barriers to attending and completing an outpatient cardiac rehabilitation (CR) program.     My 29029 Jackson Hole Journey - captured in this beautiful recap video! From the day I got the “golden ticket,” every moment has been extraordinary. This experience has not only reaffirmed my grit and passion it also revealed my conviction and ability to do hard things, accept help, and offer support. I’ve rediscovered a part of myself with an indomitable spirit to press on and a renewed commitment to being present in each moment, knowing it’s only once. What began as a deeply personal journey evolved into a shared experience that was nothing short of transformational. The bonds formed, the support given and received, and the collective energy of everyone involved turned this challenge into an unforgettable life-changing event. Colin O’Brady talks about the “1’s and the 10’s” in our lives. We don’t get to the 10’s in spite of our 1’s; we get to the 10’s because of our 1’s. I had some 1’s—enduring hard, uncomfortable moments. I persevered, and my red hat after my 19th ascent was a sweet 10, all the sweeter because of my 1’s.
