We just partnered with the Salvation Army for a quick, pop-up food distribution, getting healthy food into the hands of families who need it most. In a place where fresh options are scarce, this collaboration makes a real difference. It’s not just about handing out food—it’s about showing up, lifting up, and making sure no one is left behind. This is how we show our love! This is how we help our communities stay healthy. So, on any given day we have groups of 10-15 seniors from THE SAME MLTC or MAP...what if those providers sent ONE RN to do vital signs, answer questions or even a quick medication assessment.. ??? How many can she see in a day? They could get FIRST HAND data from their own membership in a comfortable, culturally friendly environment; It's what "value-based" contracting is to us. Can you imagine all the things we can do?
I had such a great time at today’s HSG Value-Based Contracting Seminar! The discussions around innovative approaches to care coordination and the importance of aligning incentives with outcomes were incredibly valuable. It’s clear that value-based contracting is THE future, and I’m excited about how we can apply these models to better serve our communities. A big thank you to the speakers and organizers for sharing such impactful insights. I’m looking forward to continuing the conversation on how we can create better healthcare solutions through collaboration and smarter alliances. #ValueBasedCare #HealthcareInnovation #CareCoordination #SeniorCare #Collaboration #HealthcareOutcomes
