To improve the health outcomes and daily lives of people in the U.S. with the most complex care needs, seven leading health care foundations — Arnold Ventures, The Commonwealth Fund, The John A. Hartford Foundation, the Milbank Memorial Fund, Peterson Center on Healthcare, the Robert Wood Johnson Foundation, and The SCAN Foundation — are working together to accelerate health system transformation. This collaborative seeks to help health care organizations participating in value-based payment models — including accountable care organizations and Medicare Advantage plans — adopt evidence-based interventions to improve outcomes and lower overall costs of care for individuals with complex needs. The Better Care Playbook (the Playbook), led by the Center for Health Care Strategies (CHCS), represents one step toward this aim. This online resource center provides health care stakeholders with the best available knowledge about promising approaches to improve care for people with complex needs and spread best practices.

CHCS' The Better Care Playbook is excited to be kicking off this year's Putting Care at the Center 2024 conference here in Pittsburgh! The Playbook is cohosting a preconference symposium, in partnership with Camden Coalition and UPMC Health Plan, where complex care experts from across the country are sharing insights and research on effective strategies for addressing social needs in health care and translating evidence into practice to improve care delivery. Thank you to Allison Hamblin, CHCS President & CEO, for setting the stage for today's conversations! In this first session, Karla Silverman, CHCS Associate Director of Complex Care Delivery,?is moderating a panel on what current research is revealing about supporting social care interventions through health care. Providers, policymakers, and payers are routinely supporting screening patients for health-related social needs and connecting those who screen positive to resources like housing, food, and transportation. Yet, while the importance of addressing unmet social needs is clear, the best way to intervene is not. National experts on social care programs — Caroline Fichtenberg, Co-Director, Social Interventions Research and Evaluation Network (SIREN); Pamela Corcoran, Advisory Council Member, The Center to Advance Consumer Partnership, Camden Coalition National Consumer Scholar; and Seth Berkowitz, Associate Professor of Medicine, University of North Carolina at Chapel Hill School of Medicine — are exploring the impact of these programs on patients and what the evidence shows works and what doesn’t. #CenteringCare24 #EcosystemsOfCare #ComplexCare #HRSN #SDOH

Medical respite care provides a range of essential services for people experiencing homelessness who are recovering from illness, injury, or hospitalization. These programs combine short-term housing with access to medical, behavioral, and other critical services, helping to interrupt cycles of homelessness and hospitalizations. In July 2024, the Centers for Medicare & Medicaid Services launched the Guiding an Improved Dementia Experience (GUIDE) model, a new payment approach to support comprehensive dementia care. Under the GUIDE model, Medicare Part B providers will receive a capitated per beneficiary payment for providing integrated care that addresses health care and daily life needs while also supporting family caregivers. Care management interventions that integrate community health workers can improve health outcomes. Community health workers (CHWs) who share similar health and life experiences with the people they serve can help assess and provide care for people with complex care needs. However, many community-based organizations (CBOs) employing CHWs lack the infrastructure needed to facilitate contracting with health care entities. The Pathways Community HUB Institute (PCHI) Model coordinates the administration and infrastructure for a network of CBOs and their partnerships with health care entities, with CHWs as an integral component.
