The first point of contact for patients seeking treatment for #mentalhealth health conditions is usually their #primarycare physician (PCP). About 44% of those who died by #suicide worldwide between 2000 & 2017 had visited their PCP?within 1 month of their death. The shortage of available #psychiatrists means that PCPs provide treatment & prescribe mental health care by default, especially for children, adolescents and #geriatric patients. No matter the reason for hospice discharge, that discharge leaves the family literally stranded without support. When patients and families are discharged from the hospice benefit there is a lack of information and assistance in getting the services they are losing, which may be (partially) found elsewhere. A plan of how to get them different community support needs to become the plan of care. Is it because #dementia was the main diagnosis for admission? Dementia doesn't play by the rules for determining approaching death.
