The CARIN Alliance is a multi-sector alliance led by distinguished risk-bearing providers, payers, consumers, pharmaceutical companies, consumer platform companies, health IT companies, and consumer-advocates who are working collaboratively with other stakeholders in government to overcome barriers in advancing consumer-directed exchange across the U.S. Our vision is to rapidly advance the ability for consumers and their authorized caregivers to easily get, use, and share their digital health information when, where, and how they want to achieve their goals. Specifically, we are promoting the ability for consumers and their authorized caregivers to gain digital access to their health information via non-proprietary application programming interfaces or APIs. We envision a future where any consumer can choose any application to retrieve both their complete health record and their complete coverage information from any provider or plan in the country.

    Mobile / digital drivers licenses will lead the way for other digital IDs and verified documents - including in healthcare. This will significantly shift the dynamics of healthcare interoperability over time. The pandemic fuelled a ton of innovation in this space. Especially in SMART Health Cards and Links - which are a kind of digital, verifiable health record that can be transported by end users. A balloted HL7 standard for digital insurance cards already exists, supported by CARIN Alliance. ONC's proposed HTI-2 will make the capability of generating SMART Health Cards a requirement of EHR certification. Gradually, then suddenly.

    Our research indicates half the states in the United States will have launched a mobile driver’s license by 2025 ???This means >60% of the population will have access to private, trustworthy IDs conveniently held on their device. Furthermore, recent launches of mDLs in New York and California (where adoption numbers have been published) indicate people want this! New York added 100k users in under 2 months. In California, over 100k people are downloading the mDL each month. When you look at market size (or in this case, coverage), there are 2 things that matter: 1???How big is the market now? 2???How quickly is the market growing? The absolute size of the mDL market is relatively small today. We estimate Americans hold between 5 and 6 million mDLs overall. But the mDL market is growing very, very quickly. In fact, based?**only on extrapolating historical growth rates**, we project by the end of 2026 100 million mDLs will be issued just in America. Other markets like Australia, Japan, Korea, Hong Kong, etc. are growing just as quickly.     I have so many thoughts on this and could literally write a thesis about this topic. I had testified in front of congress about these tools and the importance of not only giving them to providers, but putting them in the patients hands. It’s interesting to see the adoption from a hospital perspective as that setting was not, in my opinion, the one that would have the most impact. We still have an opportunity to get these tools in the patients hands where I truly believe they would have the most impact!     Despite being in 2/3 of hospitals, real-time benefit tools don't seem to be well utilized yet for the populations that need it most. --- Last month, the first national estimate of real-time benefit tools (RTBT) (aka RTBCs) adoption that I’ve seen was published in JAMA (in the comments). The study found that 67.9% of hospitals had adopted a RTBT, but the hospitals that could best utilize them were less likely to have one. "RTBT adoption was less common in hospitals that were small, for profit, nonaffiliated with health systems, rural, and in counties with lower household income and worse health measures." --- Another study from 2022 (also in the comments) saw that RTBTs reduced out-of-pocket costs by 11.2% overall, and 38.9% specifically in high-cost drugs. However, that study also found that only 4.2% of claims had an available alternative. The technology caught the attention of Congress and led to including RTBTs under the definition of a qualified EHR: "capable of including, a real-time benefit tool that conveys patient-specific real-time cost and coverage information with respect to prescription drugs" While the capability to add a RTBT doesn't mean a requirement, it does make the technical process easier for the hospitals, clinics, or other EHR customers to add a RTBT. --- I'm impressed that 2/3 of hospitals have a RTBT--this is much higher than I expected. One limitation to current RTBTs is displaying the net cost instead of just the list price. This can be difficult to incorporate due to the confidential nature of rebates and other contract discounts. Personally, the next steps where I'm interested in seeing more data are how are prescribers utilizing (or not) the RTBT, and what kind of effect RTBTs can have in the populations that need them most. Maybe those populations are where the 4.2% of impactful claims reside.

    ????Incredible news! At the Civitas Networks for Health conference this week, Selfii announced they are live with a “first-in-the-nation” approach using CLEAR, MedAllies, and Connie to do IAS using a single IAL2 credential! What does this mean? A consumer can prove their identity once via CLEAR and authenticate on the Selfii application, send that same digital credential to MedAllies who will find their records on Connie and return those records!! One Login! ???????? We’ve been promoting this type of an approach in the CARIN Alliance for 7 years and it’s finally here! The results? Zero mismatches which means they never returned a mismatched record to a consumer. (Yes, you read that right. Never!) More great lessons learned below. Congratulations to David Bardan, Jason Sherwin, Anil Sethi, Glenn Keet, Deven McGraw, A. John Blair, III, MD, Jennifer Searls, MHA, and everyone else who made this happen! Now, we need to do this everywhere so consumers can have seamless access to their health information from anywhere in the country.     We want to applaud Micky Tripathi and the Assistant Secretary for Technology Policy office for this outstanding blog post today. After more than 7 years of FHIR APIs being in production, our consumer-facing application community is experiencing all of these issues and more. This is severely impacting their ability to provide patients with the health care information they need for themselves and their loved ones. We also believe this is largely a behavior problem and this bad behavior needs to stop. Immediately. We have been clear for the last few years on the challenges we are facing.     ???Exciting Updates from the SMART Health Cards/Links and International Patient Summary Breakout Session at the #HL7 Connectathon!??? I’m thrilled to share some significant updates from our recent breakout session: - We announced that the?Health Level Seven International #FHIR SMART Health Card and Links Implementation Guide (IG)?has been submitted for the September Ballot cycle. You can check it out here: https://lnkd.in/eg5-Twv4 - The IG is currently passing the ballot and has received numerous comments. Next Steps:?Our goal is to work through responding to tickets with implementers and HL7 workgroups, update the IG, further test it, and then move it through the ballot reconciliation process to publication. During our discussion session, we focused on two key use cases leveraging SMART Health Cards and Links (SHC/SHL): the?CARIN Alliance Digital Insurance Card IG?https://lnkd.in/esd6YiVc and the?International Patient Summary IG https://lnkd.in/eDX6j9pA. We covered several critical topics that are essential for developing an app using these IGs, including: - Security: Ensuring robust protection with PIN codes and dual-factor authentication. - Patient Photo: Incorporating patient photos for better identification. - Image of Physical Insurance Card: Including images of physical insurance cards for easy reference. - Best Practices for QR Codes: Using logos to identify the source or owner of the QR code without obstructing it. We also had an exciting demo of a production app leveraging a SHL (QR code) to display a Digital Insurance Card. The app displayed member information, pharmacy plan details, out-of-pocket costs, and allowed users to upload an image of themselves and the front and back of their card, among other useful data. Once deployed, a patient could scan their insurance card’s QR code (within the app) when checking in at their doctor’s office–transmitting their member ID and other coverage information directly to the clinic’s system, use the self-image to identify themselves (possibly avoiding the need to share a driver's license), the provider can also capture the necessary information for proof of insurance based on the information displayed in the app. These discussions are pivotal in advancing our work and ensuring that we meet the needs of all stakeholders.     At the Health Level Seven International Connectathon, the CARIN Alliance held a breakout session to share the latest updates on the CARIN Blue Button? Implementation Guide (IG). This IG outlines the CARIN for Blue Button Framework and Common Payer Consumer Data Set (CPCDS), providing resources that payers can display to consumers via a #FHIR #API to meet part of the #CMS requirements related to the #PatientAccessAPI. ???Key Milestones: STU1: Published in November 2020, with minor technical corrections in July 2021 (STU1.1.0). STU2: Published in November 2022, including oral and vision types for the first time. ???Ongoing Efforts: In 2023, we continued reviewing Jira tickets and discussed updates to the IG to include non-financial EOBs to meet the CMS Advancing Interoperability and Improving Prior Authorization Processes Proposed Rule proposal. In January 2024, the Interoperability and Prior Authorization Rule was finalized, making over 40 references to the CARIN IG for Blue Button. ???CMS Recommendations: CMS updated its recommendation to move from the STU 1.1.0 to STU 2.0.0 version of the CARIN IG for Blue Button IG to meet the requirements of the Patient Access API. CARIN IG for Blue Button STU 2.0.0 includes dental and vision (vision as part of the professional and non-clinician profile). ???CMS Statement:?“…if impacted payers use the recommended IGs also listed in Table H3 for the APIs and follow the IGs to specification to build their APIs, they would be conformant with the technical requirements.” Oral and vision claims information is now required as part of the Patient Access API. ???Looking Ahead: Additional details about the updates can be found in the attached slide deck.
