This innovation allows safety-net organizations to segment patients directly within their EHRs and enroll those patients into automated messaging to close gaps in care. This week, we made a big announcement that will change the way safety-net organizations drive clinical outcomes for their patients and promote health equity: Automated Gaps-in-Care Journeys. Our initial release focuses on diabetes, the seventh leading cause of death in the US and a huge concern for FQHCs across the country. Not only is addressing diabetes a critical UDS measure, FQHC patients in particular are disproportionately affected by diabetes: 14% of the FQHC population is diagnosed with diabetes, compared to?11.6% of the general US population?and nearly 30% of FQHC patients have uncontrolled diabetes. By integrating directly with EHRs, this new capability of our Health Equity Engine empowers staff to increase efficiency and connect patients with diabetes to the care and critical health information they need. Senior Vice President, Partnership, Development and Innovation at National Association of Community Health Centers (NACHC)
Looking back on the National Association of Community Health Centers (NACHC) 2024 Community Health Institute & Expo and InnovationEx, I am always so impressed with how innovative, resilient and person-centric health centers are, and have always been. Last week in Atlanta, we hosted NACHC’s Innovation Advisory Council – a group of committed, mission-driven leaders from health centers, PCAs and HCCNs, who come together each quarter to discuss challenges and innovative solutions in the health center ecosystem. We are grateful to our co-chairs, Julia Liou and Faith Polkey MD, MPH, and to all our members. Last Friday, NACHC’s Council, together with the Center for Community Health Innovation, NACHC Select and many of our brilliant colleagues, hosted InnovationEx – a NACHC-members only event for over 150 health centers, PCAs and HCCNs, featuring a keynote by Adimika Arthur of HealthTech4Medicaid, a panel moderated by our CEO Kyu Rhee, MD, MPP on Advancing #TechQuity for our Nation with Janna Guinen from the HLTH Foundation and Tanisha D. Hill, PhD, MPH from the Digital Health for Equitable Health Alliance. We piloted a new workshop on the 5X’s of Health Center Innovation with Peter Shin, Amy Flowers and Camila Silva. Lastly, a new program was piloted called the #TechQuity Tank, where 4 amazing companies pitched their solutions to our panel of health center judges. Thank you Azara Healthcare and Jeff Brandes, CareMessage and Cecilia Corral, eMed and Ron Coursey, and Syntax Health and Rachael Jones, MPA, and to our judges Rashad A. Collins, Deborah Morrison, Julita Mir, MD, and to our colleagues Jason Bellamy and Camila Silva. The rest of the weekend was filled with many of NACHC’s amazing Partners of Choice on the main stage, at key meetings and other panels and sessions. My team hosted sessions with The Commonwealth Fund, the Georgia PCA, Capital Link, Mission Mobile Medical, Leon Lowenstein Foundation, Inc., CohnReznick LLP, Massachusetts League of Community Health Centers and more. Hosting Sesame Workshop thanks to Quest Diagnostics with Super Grover and the Sesame book give-a-way was a treat! We love seeing our NACHC Leader Sponsors in person. Thank you to Centene Corporation, CNECT, Forvis Mazars US, McKesson, Mission Mobile Medical, NextGen Healthcare, OCHIN, Inc. for your partnership. Day 2 of NACHC’s #CHIexpo24 and we have been INSPIRED! Our CareMessage teams came in full force ready to connect with customers, partners, and health centers who are in this healthy equity fight for the long run. We’re also so humbled to get to learn from visionary leaders like Paloma Hernandez, Kyu Rhee, MD, MPP and countless others who continue to inspire the health center movement.
