The innovative approach automatically identifies the right care state (Pre-treatment Surveillance, Active Treatment, Post Treatment Surveillance etc.) of the patient using clinical data processed by AI models and recommends the right care team MD vs. APP for an in-person, virtual or telehealth follow up. Tracking, scheduling and managing pre-treatment and post-treatment surveillance using AI based triage and scheduling ensures that clinical resources are allocated optimally and no patient falls off the care plan.
