A new paper in the American Journal of Medicine makes the case for high-quality Remote Patient Care as a cost-effective way to address chronic disease disparities in rural America. But outdated Medicare reimbursement models are holding it back. The problem: ?? The U.S. faces a shortage of up to 40,000 primary care physicians by 2036, disproportionately impacting rural areas. ?? Over 50 million Medicare beneficiaries manage chronic conditions like hypertension, diabetes, and heart failure, driving significant healthcare costs. ?? Despite the affordability of Remote Patient Care (~$600 per patient annually), Medicare reimbursement policies disincentivize its adoption, especially in rural communities. The call to action: ??Cadence is calling on Medicare to reform reimbursement models to reflect value-based care rather than outdated geographic payment differences. ??Ensure high standards for patient-provider relationships, 24/7 support, EHR integration, and measurable clinical outcomes. ??Expand access so all patients—regardless of location—can benefit from equitable, high-quality virtual care. "To deliver the same high-quality, virtual care for all patients, reimbursement should reflect the level of care being provided and not the geographic area where the patient lives," say the paper’s authors, Dr. David I. This week’s #MissionMonday highlights the provider’s perspective—how some physicians hesitate to adopt remote monitoring, fearing a loss of control or added administrative work. Dr. John Morrison of Lifepoint Health?’s FryeCare Cardiology shares his experience, emphasizing Cadence’s collaborative, team-based approach that helps triage patient concerns and ease his workload: “They leave concise notes regarding medication adjustments, flow charts with clinical data (BP, HR, and weight), and follow a vetted tiered process for upgrading their care and reach out to our office when needed. I don't get all of the calls from my sickest patients anymore and they are BETTER cared for because Cadence can respond a lot quicker than I can with a busy practice. When I see my patients back a few months later, they come in on good GDMT and feeling much better.” Dr. Morrison’s experience reflects what we strive for at Cadence—empowering providers with the support they need to deliver better care while reducing their workload.

Primary care physicians are stretched thinner than ever, making it critical to find solutions that ease their burden while maintaining quality care. Dr. Mark Carvalho, MD, MBA, CEO of Montage Medical Group, highlights a stark reality: “Primary care doctors are at an absolute premium. We bring one in, and they almost immediately fill up with new patients because there's such a gap. So, any of the tools that we use, such as [remote patient monitoring], are offsetting office visits and creating capacity because we are never going to get there from a hiring perspective.” Montage is leveraging Cadence to offset office visits, reduce in-person patient volumes by up to 1,000 visits per year, and free up physician capacity—all while ensuring patients, particularly in rural areas with transportation barriers, continue to receive essential care.
