    This past weekend at the American Heart Association’s Scientific Sessions #AHA24, Cadence presented the largest Remote Patient Care (RPC) analyses in the U.S. demonstrating significant increase in patients achieving blood pressure goal, reduced total cost of care, and hospital admissions. Results include: ?An analysis of 23,638 patients in a RPC hypertension program, 57% of whom are from rural or underserved areas, showed a 7/5 mmHg reduction in blood pressure and a 70% relative increase in the percentage of patients achieving blood pressure goal. ?A separate cost and utilization analysis, which included 5,872 patients enrolled in a RPC program compared against 11,449 patients in propensity-score matched control group, demonstrated annual total savings of $1,308 per patient. ?Cost savings were primarily driven by a 27% reduction in hospital admissions (p-value = 0.0002)– specifically, reductions in hospitalizations for heart failure and stroke. “These outcomes, achieved through a combination of virtual care, remote patient monitoring, and medication optimization, demonstrate a scalable solution to turn the tide on chronic disease management nationwide,” said Dr. David I. Feldman, MD, MPH, Massachusetts General Hospital and first author on the forthcoming study. “A strength of this analysis is the breadth and diversity of patients, healthcare systems, and geographies, including a majority of patients from rural or underserved areas,” added Dr. Marat Fudim, MD MHS, Duke University Medical Center, and author on the forthcoming study. As healthcare grapples with a surge in chronic diseases, workforce shortages, and rising costs, Remote Patient Care is emerging as a pivotal solution for accessible, sustainable healthcare delivery.     Today’s #MissionMonday spotlights Jose Rafael Ramos, a patient of Montage Health Medical Group who knows firsthand the life-saving impact of Cadence’s remote patient care. Earlier this year, as he approached his 90th birthday, Montage Health's Connected Care program helped him avoid a potential health crisis. When his blood pressure spiked, Ramos's at-home monitor transmitted the alert to Cadence, where a nurse on our Care Team assessed the situation and contacted emergency services, ensuring he received swift care. "This could have ended very differently without such a swift response," says Pablo Nicholas Veliz, MD, Ramos's primary care doctor at Montage Medical Group. "If your blood pressure readings are out of range, you're at increased risk for a stroke and heart attack. This active monitoring system gives us feedback a lot sooner so we can initiate treatment faster. It's been a game-changer."     In last week’s Sanford Health Summit on the Future of Rural Health Care, Providence’s Eve Cunningham MD MBA shared her thoughts on new strategies for transforming care delivery in rural America during a time of rapid disruption: “[Cadence] acts as an extension of our primary care team. We talk about how we need to unburden the clinicians who are in the clinics trying to get through their day, and we don’t do a good job of managing chronic diseases in a clinic visit that happens every three months. Dr. Cunningham adds, “We’ve been able to demonstrate a 3x increase in guideline directed medical therapy which is associated with a reduced all-cause mortality for our CHF patients, improved glucose management, and improved hypertension blood pressure management. And we just recently did a total cost of care analysis that demonstrates a 20% reduction in cost of care for those patients, which includes the cost of the RPM program.” This is the future of care that our rural communities deserve.     We are inspired by this video from the Bipartisan Policy Center (BPC) featuring two people whose lives have improved with remote patient monitoring, including Cadence patient David Radar. The BPC highlights two key benefits: keeping people out of the hospital and enabling timely emergency care. As David shares, "[Cadence] can get you help quickly by connecting with your doctor. You don’t have to ride in the ambulance—I had six rides in two years, and that really affects you mentally." As Medicare and other payers expand coverage for digital health solutions, more patients are using at-home tools to share health data, allowing providers to update care plans remotely.     This week, the Peterson Health Technology Institute (PHTI) published their Digital Hypertension Management Solutions report, validating the vital role Cadence is playing in transforming hypertension care, emphasizing Cadence’s clinical efficacy and long-term cost savings. The report concludes: ?? Cadence’s Medication Management solution achieves clinically meaningful blood pressure reductions compared to usual care. ?? Cost savings of $157 per member per month, per an analysis presented at last November’s American Heart Association's Scientific Sessions. ?? Underserved populations gain vital support through focused hypertension management. ?? PHTI: “Evidence supports broader adoption.” PHTI is an independent non-profit that conducts assessments of digital health technologies to help decision makers understand “what works” and “what’s worth it” in digital health.     According to the Association of American Medical Colleges, the U.S. faces a shortage of up to 40,000 primary care physicians by 2036. The shortage is particularly dire in rural areas of the country, where poor access to care already shortens the lifespans of residents. One of Cadence’s key focuses is reducing physician burden, allowing clinicians to practice at the top of their license and care for the most acute patients. This #MissionMonday, Dr. John Morrison from Lifepoint Health?’s FryeCare Cardiology shares how Cadence has help alleviate workload while delivering the kind of care he can stand behind: “At first, some of my partners were worried about having another provider assist with the care of these patients. To their approval, Cadence does a phenomenal job of keeping the provider in the loop with the patient care without overburdening the provider with unnecessary information,” Dr. Morrison shared.? “They leave concise notes regarding medication adjustments, flow charts with clinical data (BP, HR, and weight), and follow a vetted tiered process for upgrading their care and reach out to our office when needed. I don't get all of the calls from my sickest patients anymore and they are better cared for because Cadence can respond a lot quicker than I can with a busy practice. When I see my patients back a few months later, they come in on good GDMT and feeling much better.” We're inspired by physicians like Dr. Morrison, who use team-based care to reduce workload and enhance patient care, enabling clinicians to focus on their most critical patients for faster, better outcomes.

    In today’s #MissionMonday, we’re highlighting the powerful impact Cadence has had on patients like Tori and her husband in managing their health together. Their journey underscores the importance of accountability and support in maintaining peace of mind. Introduced to Cadence through their doctor as part of our partnership with Montage Health, Tori, who previously relied on a manual blood pressure cuff, found it to be a game changer. “Using Cadence makes it so much easier to monitor my blood pressure daily, and knowing that someone else is keeping an eye on my health brings me comfort,” she shared. Tori’s husband has also seen remarkable changes thanks to Cadence. “Before, when I told him his blood pressure was too high or too low, he would sometimes dismiss it. Now, he’s much more engaged and reflective about his health. If his numbers fluctuate, he actively considers what he might need to change,” Tori explained. We love to hear and see how Cadence empowers couples to hold each other accountable, delivering not just better health outcomes, but also the peace of mind that comes from knowing they’re in it together.
