??Congratulating Dr Roberto Piro and his dedicated team at Ausl IRCCS Reggio Emilia performing Italy's first Bronchoscopic Transparanchymal Nodule Access (BTPNA) procedure with the #Archimedes lung navigation system! The optimal access included a 56mm tunnel pathway to a peripheral nodule located in lower lobe, far from the airways. No idea why LinkedIn marked the video in original post as "Sensitive Content"—there isn’t even a drop of blood! ???The Vessel mapping feature allow us to plan the Point-of-entry (POE) carefully, avoiding any vessels during tunneling process. The Archimedes platform, supported by a combination of Virtual Bronchoscopy Navigation (VBN) and Fused Fluoroscopy, made this challenging approach possible. Special thanks to Florian Sagebiel for his professional support! #BTPNA #InterventionalPulmonology

?? Exciting News from Italy!???? We are thrilled to announce that the FIRST #Archimedes system in Sicily has been successfully installed at Villa Sofia Hospital in Palermo. Our heartfelt congratulations go to Dr Giuseppe Arcoleo , Dr Giuseppe La Mattina, Dr Michele Gallo as well as their dedicated nursing team—special thanks to Alessia, Marco, and Andrea—for their enthusiasm and commitment to integrating this cutting-edge technology into their daily practice, improving lung cancer management. Following comprehensive training, both the Pulmonology and Radiology teams successfully performed four cases over two days. All procedures went smoothly, including two particularly challenging cases due to very small, peripheral nodules that were nearly undetectable on fluoroscopy. One target was in a very distal area of RULB1, (#8 branch, 16 mm, 14 mm from the pleura). This patient had undergone two unsuccessful bronchoscopic procedures before. Dr. La Mattina emphasized that without the Archimedes system, reaching these lesions and obtaining a diagnosis would have been impossible. We also extend our sincere gratitude to the hospital management, to Raffaele Nanni of PulmoJ, and to Giorgio Giliberti of Pa.Ri. Consulting Srl. for their support and partnership in making this achievement possible. ?? Kudos to Florian Sagebiel and Emmanuel de Pins for their key contributions to this project!

#AirItOut Peripheral lesions in interventional pulmonology pose significant challenges due to the small diameter of the peripheral airways. Success in diagnosis hinges on precise tools and techniques. The question remains: how can we effectively reach deeper into the periphery? Recently, we had a patient with a suspicious lesion in the right upper lobe. The patient had previously undergone bronchoscopy with a peripheral biopsy, which was non-diagnostic, and was subsequently referred for further evaluation. Using the LungPoint virtual bronchoscopy system, we mapped a pathway to the lesion and employed an ultra-thin bronchoscope (MP190F) for navigation. However, upon reaching the 4th generation airway, further advancement was hindered by the narrow airway diameter. To overcome this, we attached an oxygen catheter to the bronchoscope's working channel and used oxygen to dilate the airways. In our experience, oxygen has proven to be more effective than saline for dilation, partly because saline can obstruct vision during the procedure. This allowed us to successfully navigate to the periphery, visualizing the tumor in the 6th generation airway, where a biopsy was performed under direct vision. Despite the presence of emphysema, no pneumothorax occurred post-procedure. Initial histologic analysis revealed atypical cells, with final immunohistochemistry pending. Broncus Medical Inc Olympus Medical Systems EMEA #IPE #PPL #Lungcancer
