As a non-profit with a mission to help people living with type 1 diabetes, we take a whole-person approach to care. Our program is designed to help people holistically manage their diabetes as an extension of whatever care they already receive. We’re so glad that Emilia got her strength back. 关于我们
Blue Circle Health is a nonprofit dedicated to improving the health of people with type 1 diabetes (T1D) by removing barriers and providing diabetes care, medicine, supplies, education, and support. We offer free, virtual, personalized services and support for adults living with T1D in Alabama, Delaware, Florida, Maine, Missouri, Ohio, and Vermont with additional states coming soon. Our program can include care from our doctors and nurses, educators, support coaches, and social workers. We can help with prescriptions, insurance navigation, diabetes education, and support for issues such as housing, food, and employment. Each care plan can last up to 6 months, based on the individual’s specific needs. We act as an extension of existing care teams, providing the extra support people with T1D deserve. Blue Circle Health’s work is made possible by The Leona M. and Harry B. Helmsley Charitable Trust. To learn more, visit: bluecirclehealth.org Follow us across social media at @bluecirclehealth

We love partnering with Breakthrough T1D and had a blast joining the Breakthrough T1D Southern Florida chapter for their Miami Walk this past weekend! The more spaces we can share our mission, the more people we can help in the #T1D community. We are stronger together. #diabetes #diabetesawareness #type1diabetes

Living with type 1 diabetes looks different for each individual. When someone enrolls in our program, we start with a conversation and ask them the Type 1-Diabetes Distress Assessment System (T1-DDAS) questionnaire so that we can individualize our program to meet them where they are in their journey and address their unique needs. Kathryn, LCWS, & Diabetes Supportive Counselor, shares why the Diabetes Distress Assessment Scale is so important. Learn more: https://lnkd.in/gKDkYGhc

Whether you live with type 1 diabetes or take care of someone who does, you know that T1D is truly a rollercoaster ride, filled with so many ups and downs, and we are constantly learning from it. Many members of our team live with T1D or work with people who are living with #T1D. We asked a few of them what advice they would give to someone who was recently diagnosed. Here’s what they said.

We had an amazing time connecting with our friends and community members at the Breakthrough T1D Summits in southern and central Ohio! Growing up with type 1 diabetes is tough–we get it. Our care team members, Eleni Polopolus Sheehan, a nurse practitioner, and Carrie Matuzsan, a social worker, both live with T1D and have personal and professional experience navigating the transition from pediatric to adult diabetes care. With the help of Hannah Welch, a student representative from The Diabetes Link, they highlighted how The Diabetes Link has incredible resources to support this transition, and how Blue Circle Health can make big life changes feel more manageable.
The Ohio State University chapter President, Hannah Welch (center), represented The Diabetes Link at a Breakthrough T1D x Blue Circle Health panel earlier this month! The panel focused on the transition from pediatric to young adult endocrinology!
