Here are some facts: - 86% of specialists say the lack of care coordination with a patient's other providers is a top challenge. This fragmentation makes it extremely difficult to get a full picture of a patient's health. - On average, specialists only receive 54% of the critical information they need from referring primary care physicians about a patient's medical history and reason for consultation. - For patients seeing multiple specialists, conflicting treatment plans from different providers occur 49% of the time due to lack of cross-specialty collaboration. - Communication barriers across specialties lead to care gaps, duplicative testing, and medical errors. An estimated 80% of serious medical errors involve miscommunication during care transitions between providers. - The current system incentivizes short, transactional specialist visits rather than collaboration. The median time for a specialist to confer with another provider about mutual patients is only 1-4 minutes per patient each month. The time specialists can dedicate to each patient is limited, and the current system makes it very difficult for them to coordinate with a patient's full care team. Patients with complex medical conditions suffer the most from this lack of integrated care.
