Molly is 80 years old and managing diabetes, hypertension and kidney failure. When our Belle Technician showed up at her home, Molly admitted it had been more than six months since she had spoken with a doctor about her diabetes. ? Our clinical team reached out to Molly and learned that she had a new PCP but hadn’t yet shared any information about her diabetes with the physician. She was taking insulin daily, but she was not checking her blood glucose regularly because she had an aversion to pricking her fingers. ? We connected with her health plan’s case management team who helped secure an SQ monitor for her. Molly agreed to talk to her PCP about her diabetes. She also committed to regularly checking her blood glucose using her new monitor. ? This support set Molly up for better success with her diabetes, potentially helping her avoid hypo or hyperglycemic episodes and further complications related to her condition. ? It's time for another episode of Saved by the Belle: Member Stories that Showcase Our Value and Our Mission! Episode 10: No Foot Left Unturned ? Matthew is 65 years old and juggling a lot of serious health concerns including diabetes, neuropathy and CKD. He is on dialysis and has a history of lower extremity ulcerations. ? On a recent visit, a Belle Technician discovered a wound on the base of Matthew’s left foot. He had recently discharged from the hospital and had received a house call from his physician, as well as a visit from an RN for an injection. No one had caught the wound on his foot. ? Our technician escalated the situation to our clinical team. We reached out to Matthew’s home health agency and requested wound care on his behalf. The home health nurse worked with the physician to start wound care and obtain a wound culture.? ? The culture showed multiple bacteria were present. Matthew was prescribed an antibiotic and his wound healed without further incident. Without receiving wound care and the subsequent medication, Matthew risked another hospitalization, sepsis and a possible amputation. This story is one of a million. Medicare spending on 10 diabetes drugs hit $35 billion in 2023, a 364% from 2019. This is according to a recent article in MedCityNews by Marissa Plescia. Lamont (age 69) was struggling with painful deformities and an unsteady gait when Belle arrived in his home. He had recently moved and was without a podiatrist. We helped Lamont find a new in-network provider and get him in for a much-needed appointment. ? His new podiatrist scheduled a procedure to address his right foot deformities, which were being caused by a piece of titanium from a previous surgery that was pushing up to the surface. ? A month after his corrective procedure, Lamont reported he had no pain in the foot and that he no longer needed a walker to support movement. ? We found Lamont at exactly the right time, helping him avoid falls, worsening pain and potentially a long-term lack of mobility. ? This story is one of a million.
