I recently came across the Clinical Practice Guideline for Telerehabilitation published by the American Physical Therapy Association (APTA). A very comprehensive effort outlining recommendations with supporting literature review led by a volunteer group of physical therapists, physiotherapists, amongst others. As the name suggests, the goal is to offer documented approaches to telerehabilitation in PT practice.? ? No surprise, the literature supports the orientation of limited diagnostic accuracy with telerehabilitation when compared to in-person or in-clinic assessment. This will undoubtedly change over time with the availability of new evidence-based methodologies and supporting tools.? ? What isn’t covered is the notion of #hybridtherapy, which leverages the best aspects of in-clinic therapy for advanced patient management when in a remote setting. Band Connect, Inc. is leading the way in developing the foundational clinical evidence of the value of this hybrid modality allowing clinics and clinicians to build towards a new normal that optimizes the best of in-clinic and remote rehab. Last week, I had the great opportunity to attend the PT Roundtable discussion led by Pedro Teixeira, MD, PhD and Marla Ranieri, DPT, OCS of Prediction Health and PromptEHR on the pressing issue of PT Burnout. The panelists: Kallie Slette, Nick Nordtvedt, PT, DPT, CIMT, Cert MDT, and Ben Carlson – offered an in-depth perspective on the most current issues related to burnout facing the field right now. I am excited to share my major takeaways from this event; they are as follows: 1. PTs encounter many barriers to therapeutic alliance: Whether it be time pressures from increasing administrative duties and required documentation or the general reported experience of a lack of control, PTs face a combination of mental (bandwidth and attention) and physical (time for all daily activities) obstacles to connecting with and engaging their patients. 2. Communicating regarding reimbursement practices is difficult for administrators: For clinical leaders, discussing proper reimbursement documentation and coding can be tricky for administrators. It is important to ensure that PTs are entering the correct codes for billing, especially with declining reimbursements across the board.?However, if this information is shared in a way that frames PT behavior as improper, it is likely to backfire. Rather, it is crucial to explain the necessity of proper coding as a team initiative that does not fall solely on PTs. 3. Burnout solutions exist! They just need to be utilized: First and foremost, clinic leaders need to be checking in on their clinicians. This can be as simple as a 5-minute exercise like mountain/valley/bridge or direct, brief one-to-one touchpoints. It is additionally important to encourage PTs to focus on building qualitative time with their patients for their appointments. In addition, creating opportunities for comparative and trackable feedback provides added value as well. Beyond this, seeking out technological solutions that reduce the time required for documentation and ease of patient updating is crucial in reducing time-centric burnout. Solving PT Burnout is not a one-stop shop or permanent solution fix and requires a multi-faceted approach due to the many layers of burnout, but it is addressable, as the panelists thoughtfully described. I am grateful to have attended this event and to continue to learn about the evolving nature of physical therapy.
